BACK OVER PREVENTION: SENSING SYSTEM: CAMERA
The cameras stopped working on my brand new Model Y Tesla. I filed a service request Dec 4th and it keeps getting pushed back (Currently Dec 26th with no ETA on part availability) because this part has a widespread issue on newer Model Ys. They are saying it is “Fried AP4 computer, remove and replace”. I just purchased it from Tesla in November. It only has around 1500 miles on it. I can’t use any cameras on my car including backup cameras, forward facing cameras, side cameras. No forward or rear warning or brake assist items work. It’s incredibly difficult to see out of the rear window when backing up without the cameras. I fear it’s a matter of time before I run over a little kid when backing up because the cameras don’t work. ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: WARNINGS
Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. I scheduled the soonest appointment available for 12/16 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be to dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.

Hello, I have a 2024 Tesla Model Y (Fremont Build) that was purchased in early September 2014. On or around November 30th a software update "fried" the computer according to Tesla. The call the computer AP4... Anyway, they have no ability to fix the issue because of lack of parts. They make an appt for you and then keep cancelling it. I've had two so far that they cancelled because of not having parts. There are thousands of users on Reddit complaining about the same thing. My specific complaint, is this is a safety issue for the Model Y in particular because the mirrors and rear window are inadequate for this size of a car. Visibility to the sides and rear of the car are poor when compared to other vehicles. The rear window and rear view mirror are very small. You cannot see much out of it especially when backing up. Parking in a parking lot is very problematic and dangerous because that's where people are often walking around. The side view mirrors are better than the rear view, but the driver side mirror keeps self adjusting while driving and I have to keep messing with it which makes it unreliable for use. Tesla has been awful in keeping up. Please help!

All of the camera systems are not working (inlcuding the back-up camera), and navigation has failed as well. Followed all instructions on Telsa Manual and website, with no luck. Saftey features promised by the manufacturer don't work because of this. No warnings have appeared. I have scheduled service,at the soonest available time slot of December 27th, but I'm concerned about this happening with a brand new vehicle with on 2,015 miles on it and a 12-day wait doesn't seem acceptable for something that affects safety.

There is a steering malfunction, with my Tesla Model Y it completely locks up and the steering is disabled. The steering doesn’t move at all the following warning and codes appeared on the dashboard. DI a223 Traction control disabled Drive with caution DL a222 Stability control disabled Drive with caution GTW_w074 Steering assist reduced Steering may require increased effort Ul_a020 Steering assist reduced Steering may require increased effort. Now this is the second time it has happened within few months. It was taken to the dealership with the same complaint this was their diagnoses Concern: Customer states: Feature Availability & Settings - Steering is disabled, park assist disabled , stability control disabled, traction control disabled, vehicle hold disabled Repair Notes: Confirmed customer concern. Removed and replaced steering rack, after replacement all systems operating as designed Correction: General Diagnosis Correction: Steering Rack (Dual Motor) (Remove & Replace) - Remove and Replace Parts Replaced or Added Part Quantity STEERING GEAR - LEFT 1.00 HAND DRIVE(1188832-00- A) NUT&WSHR[SW],M14X1.5, 2.00 [10],ZNFL,PTP(1109263- 00-B). There is a serious and unsafe issue with this car. A potential disaster waiting to happen.

All the cameras for the Tesla Model Y has stopped working, without this lots of features stopped working including rear camera which is needed to reverse to charge the vehicle and anyware to park. All the collision warning systems including front and side stopped working. Wipers don't activate and figured to change to manual. Headlights always stay in highbeam. Looks like there are known issues and Tesla claims they don't have hardware to replace making us to not use the car for almost 30 days.

AIR BAGS
My car was hit by another car in front left corner and side door at very high impact. Car has severe damaged and it is totaled now. Air bags on the driver side did not deploy even at such high impact. No air bags were deployed. There was only one person, myself driver, in the car. Police came and investigated the accident and filed report. I have also submitted the air bags issue to Tesla.

BACK OVER PREVENTION: SENSING SYSTEM: CAMERA
I took delivery on a 2025 Tesla Y on Oct 17. All cameras and gps stopped working on Nov 30, including the backup camera. Based on posts online, this problem is widespread but Tesla has not sent any notification. Tesla Service says the computer has to be replaced and there is a lengthy delay in getting the replacements. My appointment has been moved twice, from Dec 5 to Dec 9 and now to Jan 9. For months, many people will be driving without a backup camera in a car not designed for visibility. It’s a real safety issue and at a minimum tesla should warn current owners of the problem and stop delivering the cars that will likely be impacted. Instead, i have been unable to get any acknowledgement from Tesla that this is a known or widespread issue. They are also not making loaner cars available while waiting for the part to fix the problem. I’ve been told “it’s not a safety issue.”

SUSPENSION
On [XXX], our 2021 Tesla Model Y had a catastrophic suspension failure while driving in live traffic. The car came to a sudden stop, the front suspension was loose and the vehicle was not drivable. We had the vehicle towed to the nearest Testa service center in Burlingame, CA. Tesla service center inspected the car and found that the lower lateral links in the front suspension had failed due to two critical suspension bolts falling out. One bolt had completely fallen out and the other had became dangerously loose and was about to fall out. The bolt failures led to a catastrophic failure of the front suspension and the vehicle became undrivable in live traffic. This is a low mileage vehicle with ~29,500 miles and there was no operator error or use that would cause a catastrophic bolt failure in the lower lateral links. Images of the issue from Tesla service center show that the bolts simply fell out after not being sufficiently fastened from the manufacturer. After Tesla service center identified the problem, we researched this issue and found an extremely relevant NHTSA recall for 2020-2021 Model Y vehicles that matches the exact suspension issue that occurred to us (21V-835 / SB-21-31-003 Recall). Our Model Y was built in November 2020, which is within the affected date range of this Recall, and our issue exactly matches this Recall. Tesla service center employees agreed this Recall seemed relevant and investigated. After a couple of days, Tesla service center contacted us and claimed that while the our catastrophic suspension issue looked very similar to the Recall issue mentioned above, our specific VIN was not listed in the Recall. Therefore, Tesla service center refused to repair the failure under warranty or recall and we were stuck with massive repair bill to fix a clear manufacturer defect that is already known and has been recalled. We are not sure why our VIN was not captured in Recall, but have been affected by the same dangerous issue. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (

After latest Tesla software upgrade, all cameras and GPS stops working. All automatic functionalities of the car stops working without cameras, and it is really unsafe to drive, for example back up the car since the physical rear view mirror is very small, and car is naturally designed with those cameras to function properly.

All cameras stopped working. With this, all safety features are not working. I can’t see to back up at night. No collision avoidance, no lane departure warning, high beams don’t stay on, turn signal doesn’t stay on. It has been two weeks and Tesla has no fix. There was no warning, just noticed it one day as I was driving it and was very dangerous in dark snowy conditions. I called Tesla service immediately and they gave me an appointment for following week. Then next day I got message from them that there are a large number of vehicles having the issue and they said they don’t have a fix for it and moved my appt out 7 weeks. I have gotten no help from them and I told them I don’t feel safe driving the car. They have not provided another car.

The Automatic Pilot Computer (AP4) failed. This controls a lot of Tesla Model Y's auto pilot features but it also controls a lot of its safety features such as the back up camera, forward collision warning and blind spot warning. Another safety concerns is the GPS navigation is no longer function because of the AP4 failure. Navigation is critical in the Tesla ecosystem in order to navigate the charging infrastructure to maintain a charge during road trips. Tesla has admitted to the issue but has done nothing to fix it. I have attached screen shots of the conversation with their service technicians. There is also a picture of the AP4 short remove and replace task for service.

What component or system failed or malfunctioned, and is it available for inspection upon request: My drivere side door fails to open every time. The cold weather feature in the tesla app claims to pop open the door so you can grab the door to fully open only to get stuck closed. This also happens when attempting to exit the tesla. The passenger side drivers door also does not open every time. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Has been in for service many times. Concern does not replicate except at time of pickup of vehicle. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes failure to open door. Also if you are in full self driving and come to a emergency stop: The vehicle is suppost to open the door and put the vehicle in park. The vehicle only puts it into park and have to press the button multiple times to open the door. It gets stuck.
