I am writing to report a serious safety incident involving the rear seatbelt in my 2024 Toyota Prius, and to request that Toyota investigate this condition as a potential safety defect affecting other vehicles. On XXX at approximately 7:08pm , my daughter, age [XXX], was seated in the rear right seat of the vehicle. We were traveling at approximately 35 on city street in FAIRFIELD CA. She was belted in using the vehicle’s factory lap‑shoulder belt. Upon arrival, she adjusted the shoulder strap to gather her belongings and the seatbelt wrapped around her waist and cinched down to the point that it became dangerously tight and she was in severe pain. The belt continued to retract and would not loosen. Attempts to unbuckle the latch and to create slack by feeding the belt back into the retractor were unsuccessful. The belt remained locked tightly around her waist and could not be released. Because of the extreme tension and the location of the belt, we were unable to free her and ultimately had to cut the seatbelt to prevent further injury. This incident left visible marks/redness around her waist, and it presented a real risk of abdominal injury and impaired breathing. I have attached photographs taken immediately before the belt was cut that show how the belt path, seat back, and retractor configuration allowed the belt to wrap around and entrap my child. I am concerned that this may reflect a design defect in the rear seatbelt system and that other children could be similarly trapped and injured. - Windshield washer fluid reservoir level going low. It is available for inspection. - Safety concern: No liquid to wipe off windshield particles, dirt that may hinder driver visibility. - The issue was brought up to Jaymee of South Coast Toyota, Costa Mesa CA when Prius was taken for delivery May 16,2026. Request to top off made but not performed by dealer on same date. Per Jaymee, dealer top off liquid. - Personally filled reservoir 3 times only to see fluid level to go down. No liquid noticed underneath the Prius. - Further communicated issue with Jaymee and decided to make appointment with Tustin Toyota, Tustin CA., since Prius is under warranty. - May 30, 2026 Tustin Toyota: Prius brought in to dealership to check issue. Per report none found and did stress test. Had discussion with Bobby Ramirez-service manager. Per Bobby, they could not find anything wrong ??? - Toyota headquarters in Texas has been notified and spoke to Austin. He said a case has been opened and will get back to me this week of June 8th.

I am submitting this complaint to report an unexpected transmission shift event that occurred while operating my 2025 Toyota Prius at highway speed which created an immediate and obvious safety risk. The event occurred on XXX. While driving my Prius at approximately 75mph on Interstate [XXX], I reached toward the center console area to retrieve my sunglasses. I did not intentionally move the shifter, but the transmission suddenly shifted from Drive to Neutral. After moving from Drive to Neutral, the accelerator no longer provided propulsion. At the time, my vehicle was traveling downhill and was being followed by an eighteen-wheel tractor trailer. The truck driver behind me had no way of knowing that my vehicle had unexpectedly lost the ability to accelerate. To avoid being struck from behind, I steered left into a paved median area, applied the brakes, and was able to shift back to Drive and merge into the traffic flow. Had a paved escape area not been available, it seems inevitable that our chances of being in a fatal collision with the vehicle behind us would be quite high. My concern is that a normal driver movement (reaching for an item near the center console while driving) may inadvertently have caused the transmission to enter Neutral at highway speed resulting in the possibility of serious harm or fatality. What component failed: The rear plastic trim on my 2023 Toyota Prius cracked without any impact, accident, or external cause. The component is still on the vehicle and available for inspection upon request. Safety risk: While this is primarily a structural/cosmetic defect, cracking plastic trim that was not caused by the owner suggests a broader manufacturing quality issue. If this type of failure occurs in components closer to safety systems, it could pose a more serious risk. Confirmed by dealer: Yes. I brought the vehicle to a Toyota dealership and service center (Berkeley Toyota) for inspection. They confirmed the cracking. The vehicle was purchased at Livermore Toyota. Other inspections: The vehicle was inspected by the Berkeley Toyota dealership and service center. No other parties have inspected it at this time. Prior warning signs: None. There were no warning lights, sounds, or symptoms before the crack appeared. It was discovered suddenly with no preceding indicators.

After the car dealership completed the first tire rotations, at 30,000 miles I rotated the tires to find severe inner tread wear on the tires where the inside of the tire was wore to the sidewall while outside portion was intact. I took the vehicle to an independent shop and the right rear suspension/tire was out of specifications on camber causing said tire wear. Toyota did not manufacture the car so that camber could be adjusted. An aftermarket kit was purchased to be able to compensate for the lack of adjustment. This is a safety concern as if a person does not rotate every 5,000 miles it will lead to tire failure at highway speeds. This is a common problem while researching forums. I have contacted the manufacturer and was given a generic response. There are reports with documentation with tire failure as early as 11,000 miles. Toyota needs to be held accountable and make a remedy for this instead of offering consumers a run around and blame hitting pot holes or on tire manufacturers, when it is clearly a suspension limitation. Thank you.

These tires were purchased brand new and installed at Pep Boys on March 24th. By the 15th of this month—less than a month later—one of the tires had developed a bulge on its sidewall.bubbling on the tire sidewall . However, neither the manufacturer (Toyo) nor the retailer (Pep Boys) has been willing to take responsibility for the issue, refusing to process a return, issue a refund, or address the quality defect. I have clear photographic evidence demonstrating that this is undeniably a product quality issue!

The 2026 Toyota Prius illuminates a road distance on an unlight, rural road of 110 feet on low beam. Parked at the beginning of one white dashed center line, the low beams illuminate two additional white dashed lines. Road lines are a standard 12 ft painted line with a 37 ft interval between lines for a total of 110ft of illuminated road. The average stopping distance of the Toyota Prius traveling at 55mph is ~260 feet. I am able to see less than half the stopping distance of the vehicle. I have hit a deer twice because of the lack of road illumination. Each deer strike was with with two separate 2026 Toyota Prius vehicles. Both vehicles had the same illumination pattern. Fortunately, it was a deer strike, however, imagine for a moment if it were a human walking along the the road or a disabled vehicle. This is a safety concern and does not meet the NHTSA regulation FMVSS N. 108, summarized below. National Highway Traffic Safety Administration (NHTSA) regulations under FMVSS No. 108 mandate that low-beam headlights provide sufficient, controlled illumination for night driving while preventing excessive glare, generally aiming to illuminate approximately 200–300 feet ahead. I did complain to Maplewood Toyota about the issue and the Service Team stated they can only adjust the headlight within the Manufacturer's suggested range. This vehicle was purchased as a new, I made an assumption that everything would be properly adjusted coming off the manufacturing floor. Maplewood Toyota did not complete an inspection of the vehicle to determine "as found" headlight aiming prior to the deer strike. There were no warning lamps, messages or other symptoms related to this incident. Jeff Rabinort, Service Manager had a conversation with me following the incident. He did confirm there was a maximum adjustment that they could make to the headlight, but was unclear what that was. I encouraged Jeff to drive a 2026 Toyota Prius on an rural road for confirmation.

I was driving my 2024 Toyota Prius and was involved in a front-end collision. The impact was strong enough to cause significant front-end damage, but the front airbag did NOT deploy. I was wearing my seatbelt at the time. This is a serious safety concern because the airbag should have deployed in this type of frontal impact. If the crash had been worse, I could have been seriously injured. I believe this may be a safety defect in the airbag system.

On August 15, 2025, I was backing my vehicle when the rear view camera screen went blank, providing no image of what was behind me. With no warning and no image, I collided with a rock and landscape, causing damage to the rear of my vehicle. At the time of this incident, my vehicle had an open, unrepaired safety defect under Safety Recall 25TA13, which specifically addresses the condition that the rear view camera image may freeze or not display while reversing. I was not aware the recall existed at the time of my accident, and the recall remedy was not completed on my vehicle until February 12, 2026 — nearly six months after the incident. I filed a claim with Toyota Motor North America. They inspected the vehicle on February 11, 2026, found the camera functioning at that time (after the recall had since been remedied), and denied my claim. They cited owner's manual language advising drivers not to rely solely on the backup camera, but did not address the fact that the recall was unrepaired at the time of the accident. Additionally the parking collision warning and automatic braking should have engaged. The rock impacted the car right near one of the car cameras....so the collision warning and braking should have engaged. I am filing this complaint because I believe the unrepaired recall directly caused my inability to see behind my vehicle, and that Toyota's delay in notifying me and completing the remedy contributed to this incident. I am requesting that NHTSA review whether Toyota's notification and remedy timeline for Recall 25TA13 was adequate, and that this incident be added to the record of complaints associated with that recall.

The way the accelerator pedal sensor is positioned allows for water infiltration from wet carpet / tracked in snow and water. The component is available for inspection upon request. Once sufficiently saturated the sensor fails, and ceases function DURING VEHICLE MOVEMENT. The problem is reproducible but has not been confirmed as a pattern of failure by the dealership. As far as my recollection, the manufacturer hasn’t inspected the part. P2138 is the associated check engine code and the vehicle enters limp mode DURING TRAVEL. The check engine light (and subsequent problems) first appeared while in traffic on 2/10/2026.
