This is my 8th report. Just after my 7th report, in May 2025, I had my tires rotated at 25,000ish miles. The car still did not record that still shows the last rotation at 8,000 miles. Furthermore, I was told at Discount Tire that I needed new tires because the tread was so thin. I was aghast...a new Tesla with tires that wear out at 25,000 miles? I was told that Tesla does not provide adequate tires for the weight of the car, and that I could buy higher quality tires next time. Now I understand why I kept having flats in the right front tire, despite rotation: all 4 tires were inadequate for my car! I asked if I could wait until my next rotation and he said I could probably make it...but they can't let me leave Discount Tire with tread 4/32nd inch thick. I think I was at 5 or 6 /32nds. The price of appropriate replacement tires is roughly $1,500. Additionally, I just downloaded a software update; here's the first screen of the Release Notes: (uploaded below) Are you kidding me? They still have not fixed the cruise control, and instead of fixing the charge port, they add another step to sidestep the broken one. I'm hoping you will investigate and recall this car. Around 02/10/2026, all of ADAS functions of my 2022 Tesla Model Y has suddenly stopped working, and the rearview was just black. On 02/27/2026, Tesla service diagnosed the car and tole me that the car computer should be replaced and its total repair cost would be about $3,200. My car's mileage was 51,000 at the onset of the problem, so just passed the standard 50K warranty. My car has total loss of rearview camera (FMVSS 111 violation) and loss of Active Safety Systems (AEB/FCW), but Tesla refused to repair a federally mandated safety defect if I don't pay the full repair cost.

Yesterday at about [XXX] I was driving using FSD. I was making a left turn on a green arrow turning from [XXX] onto [XXX] . My car hesitated, then accelerated and then abruptly braked nearly causing me to be rear ended by the SUV behind me who was angrily beeping at me. I had to rapidly accelerate to avoid being hit. The left turn signal was green before during and after my turn. I believe that the sun shining brightly on the traffic light made it difficult for my Tesla to accurately recognize that the turn signal was green. I felt this was very unsafe. There are two major safety issues that came with the vehicle upon the new car delivery. The problems existed even before the delivery as I found the first issue during the new car delivery and the second issue upon leaving the delivery center. First is the non-operational FSD (Full Self Driving). FSD never worked from the beginning. The Tesla advisor noticed it at the delivery center but insisted that it will work after 10 minutes of driving, needing calibration. However, after more than several hours of driving and 35 miles of driving, FSD still doesn't work and the display says FSD is non-functional. Second is the lane departure and lane approach sensor. When the neighboring car approaches from the side, there is no alert or notification. The blind spot sensor doesn't work either. This is a big safety issue and I almost got into an accident had I looked back and noticed a car approching from behind when I tried to move into the right lane. I expalined both of these two major issues with the vehicle and left the car at the service center in Vienna, VA, expecting the repair to be done within hours as promised. However, the service technician later sent a message via Tesla app that the part is missing from the car and the part needs to be ordered. It's been a full day and Tesla is still in the process of finding the issue, which means they still don't know what the issue is. I belive this is not the quality of product and testing that has been registered with Tesla in NHTSA. This surely represents the sub-par quality product and lack of full testing that needs to be done prior to the new car delivery. The service center employee responded via app that the car needs to in repair for 5 full days and may need more time if necessary. This is definitely not the quality and experience that are advertised by Tesla.

The vehicle's Autopilot/Full Self-Driving (HW3) computer has failed at approximately 70,000 miles. Tesla Service has diagnosed the vehicle as requiring replacement of the HW3 computer. As a result of this failure, multiple safety related systems are inoperative, including collision avoidance, lane assist, lane departure warning, automatic wipers, and other driver assistance features including adaptive cruise control. The vehicle is also unable to receive over-the-air software updates, including potential safety updates. In addition, the vehicle experiences system lag, including delay when shifting into Drive or Reverse. The center display is slow to respond, and certain media and software features are no longer functional due to inability to update. This condition increases safety risk because active safety systems designed to assist with collision prevention are disabled. The failure appears to be hardware-related and not due to normal wear. Tesla has confirmed the need for computer replacement but has declined coverage as the vehicle is outside warranty. The failed component should be available for inspection upon request.

Vehicle will suddenly apply brakes while using standard cruise control for no apparent reason. This is extremely dangerous as it has almost caused several accidents on the highway. I have almost been rear-ended several times. This is a very common problem and well documented problem referred to as "phantom braking" on Teslas and something needs to be done immediately.

I am submitting this complaint to report a safety concern involving Tesla’s Full Self-Driving (FSD) system. On [XXX], while my vehicle was operating under FSD in Las vegas NV, the vehicle was positioned in a clearly marked left-turn-only lane. The roadway had visible pavement arrows indicating a required left turn. Despite these clear lane-direction markings, the FSD system proceeded straight instead of turning left. The maneuver occurred abruptly. I did not receive a clear or timely takeover warning prior to the deviation. The transition from a left-turn lane into a straight path happened quickly, leaving extremely limited reaction time (estimated less than one second). As a result of the system’s unexpected behavior, a collision occurred with another vehicle traveling straight. At the time of the incident: Weather conditions were clear. Road markings were visible. There were no obstructions blocking lane markings. I was attentive and looking at the road. The police report assigned fault to me as the driver; however, the vehicle was actively operating under Tesla’s FSD system when it disregarded the clearly marked left-turn-only lane. I am concerned that this behavior represents a potential safety defect involving: Lane interpretation errors Failure to follow traffic control markings Insufficient takeover warning timing If this issue is systemic, it may pose a broader safety risk to other road users. I respectfully request that NHTSA review this matter as a potential safety defect related to Tesla’s Full Self-Driving system. Vehicle: 2023 Tesla Model Y VIN: [XXX] Mileage: Approximately 40,531 miles Software Version: 2025.45.9 FSD (Supervised) Version: v14.2.2.4 While driving on [XXX] near the [XXX] entrance in Mission Viejo, California, the vehicle was operating under Full Self-Driving (Supervised) mode. As the freeway lane ended/merged, the vehicle unexpectedly performed a sudden full stop at freeway speed. There was no visible obstruction in front of the vehicle. Multiple vehicles behind me had to brake abruptly to avoid a rear-end collision. The system did not provide any audible or visual warning before the sudden braking. This has created a serious safety risk due to the high-speed freeway environment. This issue appears to be related to lane-ending or merge interpretation by the FSD system. The 2026 Tesla model Y was driving in FSD (Supervised) mode in the Home Depot Colma, California parking lot, located at [XXX] on [XXX] at [XXX]. It stopped to let a black pickup truck in front of it pull about 3/4 of the way into a parking place on the right side. After waiting several seconds, it turned the wheels slightly to the left and slowly crept around the pickup truck, when the truck reversed and hit the passenger side of the vehicle at low speed just ahead of the rear wheel well. I believe a human driver would have waited until the pickup truck was fully parked before proceeding, thus preventing this accident, thus highlighting a failure of the FSD (Supervised) system. There were no warning lamps, messages or other symptoms of the problem prior to the failure, The accident was reported to my insurance company and the vehicle was inspected by a collision repair shop. Component/System: Braking System (Caliper Bolt) and Suspension (Control Arm/Linkage Bolt). Description of Incident: While traveling on a tollway at 70 mph, I heard a faint noise followed by a severe metallic scratching sound coming from the front driver-side wheel. Upon inspection at a tire service center, it was discovered that a brake caliper bolt had fallen out, causing the caliper to displace and score the inner rim of the wheel. The vehicle was towed to a Tesla Service Center, where the caliper and wheel were replaced under warranty. Tesla could not identify a root cause but stated they performed a quality check on all remaining wheels. Subsequently, a second failure occurred on the front passenger-side wheel. A different bolt—this time related to the suspension—loosened or fell out. The vehicle produced a severe grinding noise and felt unstable, as if the wheel was about to dislodge. Safety Risk: Both incidents posed an immediate risk of catastrophic mechanical failure at high speeds. A detached brake caliper can lead to total loss of braking or wheel lock-up, while a suspension bolt failure can cause a complete loss of steering control. Either scenario could have resulted in a high-speed collision, endangering my life and the lives of my two children. Confirmation/Inspection: • Confirmed by Dealer: Yes, Tesla Service Center performed repairs on both occasions. • Available for Inspection: The vehicle has been repaired, but service records and photos of the damage/missing bolts are available. • Other Inspections: Initial driver-side failure was visually confirmed by Discount Tire. Warning Signs: There were no dashboard warning lamps or messages. The only symptoms were a brief "pop" or "click" followed immediately by loud metal-on-metal grinding and vibration.

While driving, the car came to a sudden stop in the middle of the road, making a loud screeching sound. The sound persisted when the vehicle was made to drive. Checking underneath, I noticed a large bolt fell off. Along with the noise, the suspension also felt very stiff while driving forward. In any case, the car coming to a halt all of a sudden in the middle of the road is a life threatening experience and needs to be looked at.

Front Control Arms were replaced in January of 2025. I replaced all 4 tires and during the alignment on February 18, 2026 the technician reported to me that the front control arms were beginning to crack. He told me it is a known issue on Tesla model 3 and model Y. I have set up a service appt to address the issue again. Of note, Tesla service centers refer to control arms as "links" which is very deceptive on the service receipt.

The contact owns a 2026 Tesla Model Y. The contact stated that while driving approximately 70 MPH, there was a loud pop coming from the rear of the vehicle. The contact noticed that the glass on the passenger’s side had shattered, and glass had fallen inside the vehicle. The contact pulled over to the side of the road and noticed that there was no impact on the glass to cause it to shatter. The contact returned to the residence and parked the vehicle. The dealer was called, and the dealer stated that the repair was not covered under recall or warranty. The vehicle was taken to the dealer, who confirmed that the failure was not caused by an impact and informed the contact that the parts were expected to be ordered and should arrive within a week for the repair. The vehicle was not yet repaired. The manufacturer was informed of the failure. The failure mileage was 13,000.

The vehicle display flashed red and then the vehicle turned off completely while in traffic with no ability to change gear, operate the electronic door releases, or turn the hazards on. Safety was put at risk because insufficient notice of failure was given so it was impossible to get out of traffic, the vehicle could not be put in neutral so could not be pushed out of traffic, further the hazard lights could not be turned on so I had to stand in traffic waiving people around the vehicle. Because the electronic door releases didn't work, the doors could not be opened from the outside, had a front window not been open, it would have been easy to trap our child in the car. The dealer confirmed that the failure took place from vehicle logs but was unable to reproduce the problem. The dealer/manufacturer inspected the vehicle. Prior to this happening the vehicle showed warnings that power was reduced and vehicle may not restart about 20 minutes prior to the event, but the warnings disappeared after a minute. Even though Tesla confirmed the failure, they refused to diagnose the problem or fix it so the vehicle remains dangerous.

When Adaptive cruise control is on random phantom braking occurs. It can be very severe and drops speeds of 20-30 miles within seconds. Occurs on highways and is random and occurs daily. ( multiple times in a day) Has been serviced 4 times and Tesla cleans the cameras and adjusts them. It has progressively gotten worse and Tesla said don't bring it in because they cannot fix it. They describe it as a software update.

Windshield developed a long crack overnight while parked in an enclosed garage with stable temperatures. No rock impact was observed or heard during driving prior to parking. Upon inspection, a small mark resembling an impact point was found, but it is unclear whether this is an external impact or a stress fracture originating from within the glass. No unusual temperature changes occurred overnight. The crack appeared spontaneously and is disproportionately large. In 15+ years of driving other vehicles, I have never experienced a windshield crack under similar circumstances. This is a widely reported issue among Tesla Model Y owners, with numerous reports of spontaneous windshield cracking with no identifiable external cause. The windshield is a structural safety component critical for passenger airbag deployment and roof crush resistance in rollover accidents. Fragile or defect-prone windshield glass poses an unreasonable safety risk.

INCIDENT INVOLVING FSD (SUPERVISED) v14.2.2.4. On February 15, 2026, while the vehicle was operating under Full Self-Driving (Supervised) v14.2.2.4, the system failed to detect a stationary curb. The vehicle initiated a trajectory that resulted in a collision, causing a structural gouge in the passenger-side rear tire sidewall. Per MA 540 CMR 4.04, this damage renders the vehicle unroadworthy. I have saved Dashcam/Sentry video footage as proof of the system's failure to navigate the obstacle.

The 2026 Telsa Model Y Juniper I took delivery of on 2/19/2026 has no emergency release in the front trunk (the frunk). This should be standard but for some reason after Oct 2025, Tesla decided not to include this in the new models. This is a safety hazard as if someone were to get locked in the frunk (front trunk) they would have no way of being able to get out.

The contact owns a 2022 Tesla Model Y. The contact stated that while charging the vehicle or using the onboard navigation system to search for a charging station, the entire infotainment central processing unit shut down unintendedly. The contact stated that there was an abnormally low humming sound, and approximately 30-seconds later the infotainment screen shut down unintendedly. The contact had taken the vehicle to a local dealer where the battery and the coolant system were checked. The contact was advised by the dealer that they were not able to isolate the failure and needed to run additional diagnostic testing. The vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 22V296000 (Back Over Prevention, Equipment); however, when the dealer was contacted about the recall, the contact was advised there was no recall associated with the VIN. The manufacturer was not informed of the failure. The failure mileage was approximately 70,000.
