while vehicle in autopilot mode and driving on 2 lane highway, vehicle would suddenly brake hard for no reason to do so. The same would occur on the freeway at the beginning of a bend in the road. This phenomenon commonly occurred and I have stopped using the autopilot feature for fear of being hit from behind if the vehicle suddenly braked for no apparent reason. No previous warning lamps or messages when the problem first occurred. I did see messages on 4/1 during my drive that some of the cameras on the car were disabled or blocked and the messages disappeared and reappeared several times for about an hour and then stopped appearing. I have not had these messages reappear since. I have a service appointment scheduled at the end of April.

Over the course of an eight hour drive on a single lane highway, the vehicle abruptly applied brakes without any warning at full speed multiple (at least 30 times). In some instances, there were vehicles in the opposite lane. In other instances, there were none. In all cases, there were no vehicles in front in the same lane. Weather conditions were clear and dry, with full daylight illumination. This issue is most likely related to the camera-only change made to Tesla vehicles, which are used for TACC and autopilot features.

My Tesla Model Y has an ongoing problem with unexpected and unnecessary automatic braking when there is no apparent reason for the system to act--there is never anything in the lane ahead to brake for. The car just brakes and rapidly slows down at highway speeds. This has not resulted in a collision yet, but I'm fairly certain that it could result in a rear end collision if the driver following my car is "tailgating." At the very least, this is likely annoying to drivers following my car when this occurs. This is a very unnerving experience and should be fixed by the manufacturer. Tesla owner forums refer to this phenomenon as "phantom braking." This only seems to occur when I'm using the adaptive cruise control and/or the autopilot features on the vehicle.I have noticed this happening often on two lane and four lane highways, but not on freeways to date. I recall this happening every time I drive for any distance using the cruise control and/or autopilot. When I turn off the cruise control and/or auto pilot, it stops.

Frequent abrupt and unsafe phantom braking while using cruise control. Mostly due to headlights from semi-trucks coming in opposite direction but occasionally from just a dark patch on the road. On a long road trip from WA to AZ, it made using the speed control feature impossible. The vehicle would lurch hard enough to physically throw you forward in your seat and rapidly slow the car (increasing potential to be rear ended). No warning messages or failure notifications. The problem was reported to Tesla (not to police, insurance, etc because no accident ensued) and the response was in a nutshell “it is working how it should “.

Highway driving using cruise control and car suddenly brakes hard. Has happened 3 times. All the incidents seem to be on slight hills with no traffic/nearby cars or road hazards. Took place in remote parts of New Mexico on Interstate 25. The braking is hard enough to cause an accident or to be rear ended due to the fast deceleration.

When Tesla infotainment system needs a restart due to malfunction (which is easy to accomplish by holding the 2 steering wheel buttons for 10 seconds), the car doesn't allow the turning signals to function. All other essential motor functions work normally, it's just the turning signals don't. As a result, there were a couple of times when I needed to restart the screen due to bugs and turn on my signals without success - since I had to turn in a high traffic street. It's great that the car allows the infotainment to be restarted, but I don't believe a safety function such as turning signals should be impacted - there should be a temporary way to keep them working to avoid this safety hazard. I love my car and love what the company is doing, but this one I had to report since I'm concerned this might impact a lot of other drivers and potentially cause injury.

While I am turning to driveway slowly on the brake all of the sudden accelerate d too fast hitting my house wall the went to driveway with very high speed crashing my safely parked Mercedes benz pushed to the garage .the Mercedes is destroyed and the home garage destroyed. Home security cameras have videos and recorded.

Cameras did not work with turn signals. WIPERS STOPPED WORKING. Then the screen went blank. I don’t know if the hazard lights worked initially when pressed. This occurred in a rainstorm going over 60mph. We almost were in a wreck due to not being able to see and needing to exit highway immediately. Stopped car. Rebooted. Difficultly opening charging dock 15 minutes later. Have called Tesla in Columbus, Ohio and left message.

The traffic-aware cruise control triggers constant forward collision warnings on my commute, and the system highlights as threats vehicles that are parked on the side of the road safely out of the path of traffic. The warning activates automatic emergency braking, which results in a collision hazard to vehicles behind me due to unexpected and unpredictable heavy braking. I stopped using the feature after the fifth such incident within the same approximate 7 mile trip. I have read about this safety issue previously and am aware that it is under investigation by the NHTSA, but did not realize it was this bad.

The Autopilot feature malfunctioned twice on one drive today, while passing slower moving large vehicles on a divided highway. As I was passing these vehicles, the adaptive cruise control rapidly decelerated, which required intervention in my part to avoid the cars behind me from hitting me. This has not been reported to dealer, has not been inspected, and no warnings occurred before, during or after the events.

While driving along on an interstate highway at night, the frame rate for the display system including speedometer and other vehicle information drastically dropped. This resulted in limited situational awareness of what the car was doing. Updates for lane changing, blind spot detection, and lane departure warnings were delayed/lagged as a result of the reduced frame rate. This did not impact how the vehicle drove, or activation of turn signals, and the lower frame rate did not result in any flickering of the display. A display reset was required while driving at highway speeds to regain full situational awareness.

I have had 2 seats from Evenflo. One purchased from Target and another replacement that they sent to me as I reported the LATCH connectors were installed backwards/upside down. I cannot attach the base to the anchors without putting them in upside down or twisting the straps. Since this is the second one, I believe it is a manufacturing defect.

This car indiscriminately applies the brakes in a very aggressive manner on all manner of roads including multiple lane divided highways. It will regularly slow from 75 mph to under 50 mph while driving on interstate highways for no reason whatsoever. I have come close to being rear ended at least 5 times and on a recent trip to Arizona and back. One incident nearly caused a multiple car accident. This is an unacceptable fault and is very very dangerous. Tesla needs to be forced to address this problem immediately. My car does this all the time. If you want to come drive it just say the word.

When driving on a two lane highway today, I had 3 episodes of the car applying the brakes to suddenly decrease speed when a vehicle was in the oncoming lane. I also had another event of emergency braking when a car was crossing the highway on a perpendicular road far ahead of my car. When I reached that road, the car had already safely crossed. Cruise control is unusable on the Tesla because of these events.

We have had issues with the “autopilot” function braking when no obstruction exists. Situations that trigger these unnecessary breaking events generally involve shadows on the roadway, visible disturbances such as reflected sunlight, or other visual circumstances that in some way impact the appearance on the roadway surface. These appearances are generally widespread on county roads and state highways, but are less frequent on the Interstate highways. Due to the severity of these breaking actions we have greatly reduced the use of the “Autopilot” feature, and now only us it sparingly on the interstate when the lighting conditions are good. Due to loss of confidence we use it very infrequently, only once every few weeks at most. These emergency breaking events are greatly unwarranted 95% of the time. In addition, we feel that the unexpected nature of these events and the severity of them have increased our risk of collision. This is due to the unexpected and unintended breaking causing an accident from a vehicle rear ending us on the highway. We have not been in a collision yet. I feel that this feature is not yet ready for release and it should be disabled or rolled back until such a time it can be trusted to not increase the risk of collision for the occupants. To answer the questions specifically: What component or system failed or malfunctioned: Autopilot / automatic emergency breaking. How was your safety or the safety of others put at risk?: Uncommanded and unwarranted emergency breaking increases risk for an accident. Has the problem been reproduced or confirmed by a dealer or independent service center?: No Has the vehicle or component been inspected by the manufacturer or others: Remote diagnostics performed by manufacture. No problem found. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear?: Other then alerting to the breaking action, no warnings have been displayed.

While using adaptive cruise control on the highway, the car applied the brakes on its own. There were no cars in front of us, nor on the side of us. We were very lucky there was no one behind us or we would have been rear-ended. We went from 70 mph to 65 mph in a matter of an instant. I was able to apply the gas to stop the braking. There were no warnings or messages ahead of the incident.
