FSD / vision system , no warnings , the vehicle is turning left and right on a straight road sunny conditions. I have a video that it wouldn’t let me attach to the case. I had Tesla service review this and they told me it’s standard behavior, this never occurred before and is dangerous and unusable. Autopilot has an issue that Tesla themself can’t detect and when I get into an accident it’ll Be too late. I’ve been complaining about this for 3 months and Tesla keeps telling me it’s standard behavior, this never happened before the car used to drive straight, brand new tires front and back , blanket and aligned the car drives straight while not in autopilot, issue is directly related to FSD and Tesla autopilot.

Yesterday evening, my wife and I had friends over to our house, and we were excited to show them our new Tesla. We offered to take them on a short drive around town using the Full Self-Driving (Supervised) feature. At approximately 8:00 PM, we entered the address of our local Sam's Club as the destination. As the vehicle approached Sam's Club, it entered the parking lot through the gas station entrance, which appeared to be the expected route. However, instead of proceeding to a parking space, the vehicle unexpectedly continued driving and attempted to make another loop toward the gas station entrance. At that moment, an SUV was exiting the gas station. The Tesla steered toward the SUV and then continued toward a brick wall where there was no roadway. It appeared the vehicle was attempting to drive into an area that was not a valid path. I immediately disengaged Full Self-Driving, took manual control of the vehicle, and reversed to avoid what could have been a collision with both the SUV and the brick wall. The occupants of the other vehicles were understandably upset because the situation appeared unsafe. This was a very dangerous situation that placed my wife, our friends, and me at risk. Had I not intervened immediately, the incident could have resulted in a collision causing property damage or personal injury. I am reporting this event so your engineering team can investigate the behavior and improve the Full Self-Driving software. I hope this information helps identify and correct this issue to improve the safety of the system for all Tesla owners and the public.

My 2023 Tesla Model Y experienced a serious front suspension/control-arm fastener failure while I was driving on a road trip from Phoenix, AZ to Los Angeles, CA. The incident occurred in Riverside, CA. The vehicle has approximately 40,000 miles and is still within Tesla’s Basic Vehicle Limited Warranty. One front A-arm/control-arm bolt came completely out, and another related front suspension bolt was found loose. The tire and wheel are not damaged and are not flat. The vehicle became unsafe and undrivable. Tesla’s app initially categorized the issue as a flat tire, but this is not a tire issue. It is a front suspension fastener failure that could have caused loss of control, especially at highway speed. I am aware Tesla previously issued Service Bulletin SB-22-31-002, also associated with NHTSA Recall Campaign 22V895000, involving certain Model Y vehicles where front suspension lateral link fasteners may not have been properly attached. Tesla reportedly limited that campaign to a small affected population, and my VIN was not included. However, my vehicle appears to have suffered a very similar front suspension fastener failure. I am asking NHTSA to review whether this issue may affect vehicles beyond the originally identified population. Tesla should inspect the front suspension fasteners, control arm/lateral link mounting points, torque condition, threads, steering components, and alignment, and should determine whether the failure was caused by improper torque, defective fastener retention, assembly issue, or another safety defect.

My Tesla has repeatedly exhibited dangerous behavior while Full Self-Driving (FSD) is engaged. On multiple occasions, the vehicle has stopped for a red light and then proceeded through the intersection while the traffic signal was still red. This has put my family and others at risk on a public roadway and creates a serious safety concern. One documented incident occurred on June 21, 2026, while my family was in the vehicle. During that event, the car stopped at a red traffic light and then proceeded to attempt to run intersection even though the light remained red. This was alarming and dangerous, and it required close driver supervision because the system did not behave in a safe or reliable manner. This was not an isolated incident. My Tesla has run red lights in this same manner on multiple occasions while FSD was engaged. In addition, the vehicle will suddenly stomp on its brakes on a public roadway, which is another serious and unpredictable safety issue. I have taken the vehicle to Tesla regarding these issues, and I was told that this behavior is considered normal and that nothing would be done about it. Tesla’s response was that the software is still being developed, and that this is essentially part of the risk of using the software. These repeated failures involve traffic signal compliance and erratic vehicle behavior that could easily result in a crash or injury. My family has been placed in danger by these events, and I believe this is a serious safety defect that needs to be investigated immediately. I am requesting that NHTSA investigate Tesla’s FSD system for repeatedly stopping at red lights and then proceeding through the intersection while the signal is still red, as well as for sudden unnecessary braking events on public roadways.

On Saturday, [XXX] at around [XXX] going west on [XXX] toward [XXX] stopped at the intersection of [XXX]. While driving my 2022 Tesla Model Y, Performance, I was stopped at a red light. When the light turned green and I began accelerating normally through the intersection, both passenger-side doors unexpectedly unlatched at the same time. The windows on both passenger doors lowered slightly, and the vehicle displayed a warning indicating the doors were open and to drive carefully. This created an immediate safety hazard, as the doors opened while the vehicle was in motion without any intentional action from the occupants(there were 0 occupants there). This is the second time this exact issue has occurred under similar circumstances, with the previous incident also happening as I accelerated from a red light. The vehicle should not allow multiple doors to unlatch while driving. I have reported the issue to Tesla for inspection and request that this incident be documented and investigated as a potential safety defect. I was going less that 10 mph when this occurred. Thankfully, my toddler was not sitting in the back seat and there were no passengers in the car. No injuries. This week, I was driving my Tesla at approximately 65 mph in the middle lane of a six-lane freeway. Autopilot was engaged. Traffic was moving normally, and I was not aware of any obstacle or hazard that would have required emergency braking. Without any warning, the vehicle suddenly applied the brakes and rapidly decelerated until it came to a complete stop in the middle travel lane of the freeway. I was not pressing the brake pedal. The event occurred unexpectedly and created a potentially life threatening safety hazard. my vehicle came to a stop in a high-speed traffic center lane! After I disengaged Autopilot and pressed the accelerator, the vehicle responded normally. Immediately following the incident, the display showed the message “Autopilot unavailable.” That message later disappeared, and no additional warning messages remained on the screen. I contacted Tesla to report the incident. Tesla reviewed the vehicle’s logs remotely and later discussed the results with me by telephone. During that conversation, I was told that the event was considered an “anomaly.” I was also told that the vehicle logs reflected the event approximately one hour from when it actually occurred and that the logs indicated the vehicle was not operating in Autopilot at the time of the incident. This information was inconsistent with the event, because Autopilot was engaged when the sudden braking occurred. I could not even move to berm. Tesla did not request that I bring the vehicle in for a physical inspection. Instead, I was told there was no reason to have the vehicle examined. During the same conversation, I was encouraged to consider subscribing to Tesla’s Full Self-Driving (Supervised) system, which requires an additional monthly subscription, rather than continuing to use autopilot as a result of a question I asked about two plans. WHAT HAPPENED: On June 25, 2026 at approximately 4:17 PM, the vehicle was operating under Full Self-Driving (FSD) navigating into a residential driveway. The driver pressed the gas pedal to take manual control. At the exact moment FSD transferred control, vehicle telemetry confirms the steering wheel was at −299° (nearly 3 full turns of hard left lock). No warning was given. With a ~13:1 steering ratio, approximately 300° of wheel rotation was required before driver input would affect the front wheels — taking 0.4–0.5 seconds during which the front wheels remained pointed left despite the driver turning right. The vehicle struck a tree at approximately 13–15 kph. Secondary failure: FSD re-engaged once during the first override attempt, requiring a second override before full control was obtained, further delaying driver response. Telemetry confirmed: factory crash algorithm (RCM_CRASH_ALGO_WAKEUP_EVENT_ACTIVE), Electronic Stability Control, and Traction Control all activated at impact. Post-impact speed registered −1.67 kph (backward bounce), confirming direct collision. SAFETY RISK: FSD handed off control mid-maneuver at extreme steering lock with no warning, creating an unrecoverable situation in a confined residential driveway. PRIOR WARNINGS: None. While driving my 2026 Tesla Model Y in June 2026, the vehicle suddenly applied steering input on its own and began swerving left and right even though there were no obstacles in the road. I was driving on a straight section of roadway, centered in my lane, when the steering wheel started moving by itself as if the lane keeping system and steering were confused. I had to fight the wheel in both directions to keep the car from veering toward the curb and toward the adjacent lane. It felt like struggling against another person at the wheel. Fortunately there were no vehicles directly beside me at that moment, so I was able to avoid a collision, but this behavior could easily cause an accident for a typical driver. This incident is part of an ongoing pattern of safety issues with this vehicle. Since late 2025, I have experienced multiple events where the car fails to respond to the accelerator after shifting into Drive and behaves as if it is in neutral until I shift back to Park and then into Drive again including a complete loss of propulsion while driving in moderate traffic. Tesla has already performed recall repairs on the battery/battery contactors and multiple diagnostic visits for intermittent loss of propulsion and steering concerns, but the problems continue. I am filing this complaint because I believe there is a serious defect involving the steering, propulsion system, and lane keeping driver assist systems that could cause loss of control and a crash.

On June 23, 2026, at approximately 7:45 AM Central Time, I was operating my Tesla in full manual drive mode on US Highway 35 westbound between Denton and Fort Worth, Texas. No driver assistance, autopilot, or automated steering system was activated or engaged by me at any point prior to or during this incident. Without warning and without any driver input, the vehicle autonomously activated an automated steering and driving assistance system, assumed full control of the vehicle, and did not return manual control to me despite my attempts to intervene and regain control of the vehicle. The incident occurred during active rainfall and wet road conditions. The vehicle, while operating under its own autonomous system control without driver consent, drifted and struck the guardrail on the driver's left side. Following the initial impact, the vehicle continued moving and did not stop. The automated system remained active and did not disengage following the collision. The vehicle did not return control to me until a wheel separated from the vehicle. This incident was not caused by driver error. The vehicle's automated system activated without driver input or consent, overrode manual driver control, prevented the driver from regaining control of the vehicle, caused a collision with the guardrail, and continued operating after impact until mechanical failure occurred in the form of wheel separation. I am requesting a formal investigation into this uncommanded autonomous system activation as a safety defect. This type of failure poses an extreme and immediate safety risk to drivers, passengers, and the public.

The contact’s wife owned a 2022 Tesla Y. The contact stated that while the vehicle was parked, his wife shifted the gear shifter into drive(D), and the vehicle independently accelerated. The Autonomous feature was not engaged. The brake pedal was depressed; however, the vehicle failed to respond as needed, and the front center of the vehicle crashed into a pile of cinder blocks, drove through a gate, and crashed into a dumpster, where it came to a stop. As a result of the impact, the front driver's side tire detached, and the dumpster was damaged. The contact stated that another person helped remove the driver from the vehicle due to the front driver door being unable to open. The air bags deployed as intended. No warning lights were illuminated. The vehicle was towed to a tow yard and was deemed a total loss. A police report was filed. The driver had pain in the back and sought medical attention. The contact's wife was informed of a slight fracture of an unstated bone in her back and ribs, and had to return to the doctor several times for medical assistance. The contact was certain there was no user error to have caused the crash The manufacturer was notified of the failure. The failure mileage was unknown.
