Humming sound coming from driver’s side headlight as soon as unlock bottom pressed in key fob. This sound comes from a faulty cooling fan inside the headlight and is a known defect. Humming sound progressed to very slow pitch indicating that fan function is diminishing over time. Mazda will replace the headlight but required me to pay $200 toward it. This should be a recall with replacement at no cost to the consumer as they are aware of the defect.

On 06/18/2026, the vehicle displayed multiple safety warnings, including ABS malfunction, dynamic stability control/traction control malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and airbag/seat belt pre-tensioner malfunction. These warnings involve braking, stability control, collision avoidance, airbag, and seat belt safety systems. This was not the first occurrence. On 04/24/2025, the vehicle displayed similar warnings, including critical brake system/Electronic Brake Force Distribution malfunction, ABS malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and keyless entry malfunction. The vehicle was towed at that time. The vehicle was inspected by an authorized Mazda dealer in June 2026. The dealer replaced the battery under warranty, but I was not provided diagnostic trouble codes, a root-cause explanation, or written confirmation that the braking, ABS, airbag/seat belt, stability control, Smart Brake Support, Forward Obstruction Warning, and forward sensing/camera systems were fully diagnosed and verified safe. The dealer inspection report also listed the mileage as “No Mileage.” The vehicle is available for inspection upon request. I do not feel safe driving it because the repeated warnings involve braking, collision avoidance, stability control, airbags, and seat belt systems. I am concerned that an electrical, braking, or safety-control defect may disable safety systems while driving.

About a week ago, a "telematics error" message displayed, a single red light on the SOS button turned on, and it started beeping loudly. This has repeated multiple times. In this state the vehicle cannot automatically summon emergency help following a crash, and I cannot manually trigger an SOS call. More recently, June 17th, a new symptom has occurred. Vehicle's touchscreen display would not turn on. This display is required to access heating/cooling features including defrost/defog, and is used to show the backup camera. The dealer has informed us that these issues were all caused by a manufacturer-initiated over-the-air software update. We are currently scheduled to have a new software update installed to replace it on June 22nd.

I leased a brand new vehicle on June 8th. On June 12th, I took it to the dealer's service center because of a warning message, accompanied with a loud chime that said: "Emergency Call Unit Malfunction", the SOS red light was also on. I picked the car up today, June 15th. Shortly leaving the dealership, I was driving on the freeway crawling traffic with my husband and 4 year old child in the vehicle. Suddenly, the service/wrench icon started flashing on the dashboard, and the car's engine completely died and stalled while we were moving on the highway. At the exact second the engine died, dash and infotainment screen went completely black, the A/C shut off as well. I instantly lost my speedometer and all safety alerts while stuck in traffic. I was still stepping on my brakes, and turned my hazard lights on so we could pull over to safety. Screens went back on and engine came back on. We were fortunate to be by the nearest exit and took it. The screens went back on, but service light was still flashing. We were able to drive the car to Mazda, and was provided a loaner car. This is a massive safety issue and hazard that put my family in immediate danger on a busy highway just following a dealer service.

On May 21st , 2026, while waiting in line to make a right turn (positioned 3rd in queue), the vehicles ahead moved forward. As I transitioned from the brake to the gas pedal to follow, the vehicle experienced an instantaneous engine stall and total electrical failure (NOT an i-Stop event). Pressing the gas caused the vehicle to slowly roll backward. When I immediately tried to reapply the brakes, the brake pedal became extremely stiff and nearly impossible to press down, failing to hold the vehicle effectively. The driver behind me began honking aggressively due to the imminent collision risk. I attempted to lower the windows to signal the other driver, but all electronic controls were completely dead. I had to quickly shift the gear into Park (P) and hit the start button to successfully restart the engine, which temporarily restored functions.

The passenger seat occupancy sensor goes off intermittently, causing an extremely distracting seatbelt alarm in the vehicle during operation. This has occurred to me while driving on a highway; I deeply lost focus on controlling the vehicle as the alarm started suddenly, I shoved the one water bottle off the seat to no avail, and I had to reach way over to plug the seatbelt in to silence it. In other instances where this occurs, only a full restart of the vehicle will reset the sensor. Existing TSB 08-001/23 was deemed to not cover my vehicle by the manufacturer, despite my VIN being in range and symptoms being identical.

My 2025 Mazda CX-5 includes a Traffic Jam Assist (TJA) stop and go feature. This feature allows the adaptive cruise control to move the vehicle in traffic with minimal drier intervention. As traffic advances the vehicle moves. If traffic stops, the vehicle stops. When clear of vehicles based on the set safe space setting in the adaptive cruise control, it resumes the predetermined speed set in the adaptive cruise control. The problem: While in "traffic jam assist" (TJA) mode, if changing lanes L or R to a lane that has vehicles farther away (clear lane), the cruise control radar/camera then sees a clear opening and begins accelerating to the predetermined speed on the cruise control. However - at this time the system fails to recognize a that another vehicle is farther ahead. maybe 4 or 5 car lengths ahead, and continues accelerating dangerously close to the next vehicle (I cannot say if it will stop or not because I have intervened to apply the brakes), cannot risk a rear end to another vehicle.The system fails to recognize that another vehicle is present after a lane change has occurred and braking or slowing as expected does not take place. This is a common repeatable scenario. The failure to recognize another vehicle after changing lanes is a serious and dangerous problem that should not be taking place. This poses an accident risk from an automated system that has a inherent flaw in its logic/design. The NHTSA should investigate this system and determine what corrective action can be taken to prevent a collision while using this system.

An initial repair and parts request was submitted to Mazda several months ago, but no corporate response or resolution was ever received. The vehicle and component are currently available for inspection at Yark Mazda in Toledo, OH, where technicians have confirmed the headlight motor failure. Because the prior request went unanswered, the dealership has advised me to contact Mazda corporate directly to follow up. No dashboard warning lights appeared prior to the component failure.

Complete software issues… 1-Yellow triangle with a red exclamation point 2-SOS alarm tone continuously goes off 3-Radio screen will freeze up, stop and restart to clear 4-Hey Google commands, will not turn AC off, Hey Google turn AC down, responds with turning the AC UP 4 degrees 5-Software turns off traction control 6-Extremely dangerous when Google does not work, and you have to look at the screen and not the road 7-Mazda has removed physical knobs for entertainment system, and AC system. With a failure of software this is dangerous 8-Why did Mazda put this model out for sale to the public, and NOT NOTIFY CUSTOMERS OF THIS POTENTIAL DANGEROUS ISSUE AT THE TIME OF SALE 9-In 2 months I have had to go back for service 3 times, with as many as 5 warning lights on the Custer at 1 time, all regarding software issues 10- Each time from the service department we get the… Mazda is aware of this issues but does not have a fix at this time 11- Paid $40,000 cash for this 2026 mazda cx5 premium, Is and or will this be considered lemon law in the state of Florida. 12- As of now the only fix is a hard reset by disconnecting the battery.

My car caught on fire on [CCC] and it's completely burned down, a total loss. Following is information about my car and some summary of what happened. Car information: - Mazda CX5 2024 - Bought in June or July 2024(car payment only made 22 times so far) - Odometer 24,445mi(as of 05/15/2026) What happened: - In [XXX] Friday afternoon, I was in Bellevue and noticed an engine overheat warning. So, I stopped immediately to cool down and called Mazda Dealership(Lee Johnson Mazda) to schedule a service. - Then after 20-30 minutes, I started the car again and the warning was gone. So I drove slowly and got on [XXX] to head home in Kirkland. I was already driving slowly. - After a few minutes, overheat warning as well as engine warning started firing and noticed that smoke coming out of bonnet. - So, I pulled over on the shoulder near [XXX] and called roadside assistance. But even before I get connected to the roadside service, engine caught on fire and I had to call 911. - The car is completely burned down and it's a total loss. - State patrol wrote me a "Exchange of Information" and "Tow/Impound and Inventory Record". Why I am submitting this form: - This car is less than 2years old and has been seeing Mazda service regularly. The odometer checked by dealership on 05/15 is 24,445mi which is slightly more than 24,000 which is stated in the Lemon LAW. But still, the car is pretty brand new and around 24,000 ish. So, I'm wondering if this still qualifies for Lemon Law. - Even before this happened, I had a service history for engine warning and get the engine checked on 05/15/2026 which is exactly a week ago before the accident. Also, they said the engine thermostat had an issue and replaced and covered by Mazda's warranty. - Overheat and engine warning was on and off. As soon as I pull over, it caught on fire. - This happened on [XXX]. Depressing the accelerator pedal fully does not disengage the automatic braking, stripping the driver of vehicle control and exposing the vehicle to a severe risk of being rear-ended at high speed. Four additional vehicles were damaged from the impact with one vehicle deemed a total loss. The right front end was impacted including the hood and the right wheel. No additional property damage. The vehicle came to a stop in the parking lot from impacting the vehicles. Both front driver and passenger side air bags deployed. The vehicle was towed to a tow yard. The vehicle was deemed as a total loss by the insurance company. The contact sustained injuries left broken clavicle, left arm, left and right knees received lacerations and lacerations and 3 broken ribs on the left side and hematoma and received medical treatment at the emergency room intensive care. A woman in the back seat of one of the parked vehicles sustained minor injuries and received medical treatment. The dealer nor manufacturer was notified of the failure. The failure mileage was 2,837

Description of Safety Defect: The driver-side exterior rearview mirror vibrates excessively, escalating into a violent shake depending on vehicle speed, crosswinds, and road surfaces. The issue is highly pronounced at highway speeds (65+ mph) and deteriorates aggressively as speed increases, particularly on open, wind-swept highways with like MA-3. The resulting violent vibration causes severe visual distortion, creating a blurred, "low-resolution" image that makes it incredibly difficult to accurately judge the distance, speed, and presence of overtaking vehicles. When checking the driver-side blind spot, this rapid blurring induces sudden, brief physical dizziness and vertigo, posing an immediate safety hazard during high-speed lane changes. The passenger-side mirror remains perfectly stable and clear under identical driving and road conditions. Attempts to Correct / Dealer History: The vehicle has been taken to an authorized dealership (Tulley Mazda, Nashua, NH) twice for this specific issue: On 03/18/2026 (Mileage: 5,862), Repair Order #XXX/1 noted the complaint. The technician verified the vibration on a road test but dismissed it as a "normal characteristic of the vehicle". On 04/21/2026 (Mileage: 6,846), Repair Order #XXX was opened because the condition worsened and began occurring at lower speeds. A ride-along was conducted, and the Shop Foreman confirmed the defect. Upon mirror disassembly, the technician found "loose internal components causing the issue". Current Status: Under warranty, the dealership replaced the body door mirror assembly (Part KJL1-69-181C) and the mirror glass/holder (Part TK71-69-1G7). However, the violent shaking remains completely unresolved and continues to worsen at high speeds. The dealership is now refusing further diagnostic or repair work, claiming the vehicle is "within spec," despite the persistent visual distortion and subsequent driver vertigo. No crash yet, but the defect is serious safety concern.
