- #11650641March 25, 2025FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS, LANE DEPARTURE: WARNING
The weather was heavy rain. The road was a two lane rural roadway with a 35mph speed limit. There were no other vehicles present and there were no obstructions in the roadway. As I slowed approaching a stop sign, the collision warning system activated. Traction control activated. The vehicle began braking. The transmission put itself in Park at ~15 mph. When the vehicle stopped moving it turned itself off. The vehicle came to a stop in the lane it was traveling in at the time it began to malfunction. The vehicle transmission would not come out of park. When I attempted to restart the vehicle it activated the horn until it was turned off again. Safety issue was the vehicle sudden stop and coming to rest in the travelled portion of the roadway. The vehicle was towed to Battlefield Ford in Culpeper VA on the date of the incident ([XXX]) and it was returned to me on [XXX]. What component or system failed or malfunctioned, and is it available for inspection upon request? When the brakes were applied, the vehicle accelerated instead of slowing down/stopping. The vehicle is available for inspection and we are begging for someone to investigate this as we do not want to get behind the wheel of this truck if the issue is determined and rectified. How was your safety or the safety of others put at risk? Because of the above issue, the car accelerated and crashed into a building putting the driver and passenger at risk. The airbags deployed. The vehicle went over a walkway where, luckily, no one was walking or they would have been seriously injured or potentially killed. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Ford keeps telling us to work with our insurance company. The insurance company did an estimate for the repairs, but they are not able to inspect the vehicle and determine the cause of the malfunction. Our insurance adjuster said that Ford needs to be the one to access the black box and perform testing to determine the malfunction. Ford has been unresponsive to our claim and the service manager where the vehicle is sitting will not touch the truck due to it being a Ford liability. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, the vehicle just suddenly accelerated when the brake was applied. The airbags deployed when the truck hit the building, the horn went off and wouldn't stop, and the brakes are stuck (unable to apply pressure).

My vehicle has a broken backup camera (extremely blurry, barely or unusable). It has not functioned properly since the vehicle had about 2,500 miles on it, around 15 December 2024. It now has 7,300 miles on it and the manufacture has not remedied the situation. The backup camera is currently on back order (since January 2025) by the dealership. The dealership implied that it was likely on backorder because there was a high demand for them since they were breaking frequently.

The component that is malfunctioning is the LCD dash screen that contains all of the dash indicators (Speed, RPM, gear indicator, etc.). The issue is that without warning, different colored lines flash across the screen. It is distracting and I am quite worried that if the screen goes out, I will not know how fast I am going, and I won't be able to see any of the dash indicators related to the system status. It is getting worse every day that I have the vehicle, and now the whole screen is flickering. I purchased the vehicle approximately 3 days ago and the issue has recurred every single day and I have taken a video every time it occurs (as of this report I have 7 videos). The vehicle has not been confirmed by the dealer or others because the dealer will not let me bring it in until their next available appointment on 3 March 2025. There are no warning lamps or messages that come up. Typically the symptoms appear after the vehicle is fully warmed up, or if the heater has been running for a while.

- #11638045January 23, 2025FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS
I was involved in a rear-end collision caused by a vehicle abruptly moving into my lane while traveling at a slower speed. From the time the vehicle entered my lane to the time of the collision was approximately one second. Despite this timeframe, the anti-collision warning system and automatic emergency braking (AEB) functionality in my Ford vehicle did not activate or alert me before the impact. Additionally, I have observed that the adaptive cruise control and Blue Cruise systems in my vehicle do not react until a vehicle in front is fully out of the lane (for acceleration) or fully in the lane (for deceleration). This delayed reaction may have contributed to the system’s failure to engage in this incident. I believe this represents a potentially unsafe functionality in the collision avoidance system, as it may not adequately respond to rapidly changing traffic conditions. I am reporting this to ensure it is investigated to help improve the safety of these systems and prevent similar incidents in the future.

While driving, the screen in front of the driver (dash) which displays information such as speed, transmission gear, parking brake, and other important information, will flicker and/or be distorted. This results in driver distraction and may prevent the driver from seeing safety-related information such as vehicle speed while operating the vehicle. The issue is intermittent, but typically resets when the vehicle is powered off and back on.

Every day at random when I drive to work I notice in what seems milliseconds that the digital gauge cluster screen will shut off and back on almost instantly, but slow enough that I notice it. It is always random and I have no way of replicating it. I noticed it after about 2,000 miles on this brand new truck.

Purchased vehicle on [XXX]. The vehicle was brand new and had 12 miles on it. I drove the vehicle home. My family and I went out to dinner and returned home and the vehicle remained in our garage. In the morning, I drove the vehicle to the gym. the vehicle was at that location for approximately two hours. I then drove it to my workplace. When I pulled into the parking lot I noticed some smoke from the hood. I turned off the vehicle and popped the hood. I then tried to open the hood, but realized that the vehicle was on fire. I backed away from the vehicle and called 911. The vehicle was ablaze within 1 minute of me exiting the vehicle. Firefighters responded approximately 8 minutes later and put out the fire. I do have videos of the fire happening INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My 2024 Ford F-150 Raptor V6, a brand-new vehicle purchased on [XXX], has accumulated 767 miles. On the evening of [XXX] in Orange County, California, while driving on the freeway at approximately 75 miles per hour, I experienced an unexpected issue. Everything appeared normal, and there were no warning lights on the instrument cluster indicating any problems with the truck. However, a few miles before exiting the freeway, I received a message through the Ford Pass App, accompanied by a warning on the instrument cluster that read, “Drivetrain malfunction, reduced power.” Despite the warning, I continued driving cautiously until I reached my home. Upon pulling into my driveway, I scanned my vehicle for error codes, which revealed the following: P0929-77 Gear Shift Lock Solenoid/Actuator Circuit "A" Range/Performance U211B-87 Control Module Message Authentication B1578-89 Digital Park Assist Camera Vision System U023A-00 Lost Communication With Image Processing Module A U0415-00 Invalid Data Received from Anti-Lock Brake System (ABS) Control Module "A" U102F-00 Invalid Internal Control Module Monitoring Data Received From All Wheel Drive Control Module The code that triggered the check engine light, as indicated by my vehicle’s scanning app, is P0929-77. On December 31st, I took the vehicle to Caruso Ford in Long Beach, California, for diagnosis. Unfortunately, Caruso Ford is currently unable to pinpoint the fault. The tech who examined my vehicle believes it’s an electrical issue rather than mechanical. I hope to receive an update on my vehicle by Tuesday, January 7th. Ford issued a recall for Ford Lightnings produced from November 2023 to February 2024, for the front ball joint nut falling off. Causing a loss of control, suspension and all steering input, later crashing. New truck with 700 miles. Fords guesstimate of 12000 trucks, going by the month math is more likely than not well over 24000 affected vehicles. MY TRUCK WAS MANUFACTURED JUNE 2024. My husband MOVED/unscrewed the aforementioned nut, off the left upper ball joint WITH THE FINGERS ON HIS HAND. The nut was loose. The upper ball joint appeared hand tight. FORD NEARLY KILLED US. I advised william at NHTSA and he hung the phone up on me. William needs to be terminated. William is apparently biased against gay men and/or retired law enforcement officers.

After having the vehicle serviced by authorized dealer the brake system and electrical system displayed the same issues as it had previously. The safety braking system sent warnings and failures 3 times and I returned it to the dealership for further repairs. While driving the ABS senses a failure, along with hill assist, and the front end collision system. All of these systems sent failure warnings and made braking very difficult.
