- #11570906February 8, 2024SERVICE BRAKES, BACK OVER PREVENTION: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Component/System failed: Brake Servo part number 1eb-614-106-t Safety put at risk: Car was unable to move and completely locked up while in "Drive". If this had happened at a stop light or in traffic, it would have dangerously obstructed traffic, as the car could not be put into neutral or any other gear and could not move. Tow truck with dollies was required to move vehicle. A flatbed could not be used since the wheels were all locked up. The vehicle has been inspected by the dealer's service department at the Audi dealer. This is a video of the warnings: [XXX] While stopped, but still in Drive mode, in a drive through lane of a fast food restaurant, the car's check engine light turned on, and the car shut off. When the the car Start/Stop button was pressed to turn the car back on, many warning messages were displayed on the dash, and the car refused to shift into Drive, Neutral, or Reverse. The service department at the dealer has had the car for over a month (Since [XXX]) , and is unable to get the needed parts from Germany. The contact owns a 2023 Audi Q4 E-Tron. The contact received notification of NHTSA Campaign Number: 23V842000 (Electrical System) however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that he had plugged the compact portable charging system into a 220/240V wall socket that licensed electricians had installed. The contact stated that after returning to the residence a short time later, there was a burning plastic odor inside the garage. The contact checked the charging cable and the wall socket and became aware that the socket was partially melted. The contact disconnected the portable charger from the wall socket. The contact stated that the 220/240V wall socket was no longer operable. The manufacturer was made aware of the issue. The failure mileage was approximately 1,500. VIN tool confirms parts not available.

I was hit by a large truck on the driver side as I was in the right lane catching up to a safe speed. The hit spun the car around at least 180 degrees. Due to the massive pain I couldn’t move. The car just shut off completely to include all functions. There were no prompts from the car’s emergency to call for help which I find as a fault. Luckily I was in a populated area and emergency was able to respond. I am seeing reports in forums with photo evidence that the SOS function is malfunctioning. The car is likely in a junk yard. I took the car in a month or so back for a charging module fault. Audi in Lehi claimed to have seen a significant number of faults but couldn’t identify and remedy them.

- #11562275December 28, 2023VISIBILITY/WIPER
The warning labels that are posted on the front of the sun visors are a visual distraction that diminish the forward sight line of the driver. When I cover the warning label with my hand the depth of field and forward sight line is greatly improved. I removed this label from previous vehicles that I owned. However, the sticker on this vehicle is part of the visor and can not be removed. I have never read this warning label, nor has anyone in my family. In my opinion, all warnings and precautions should be placed in the owner's manual. It is impossible to place every precaution or warning on a sun visor label. The label diminishes the drivers visibility and is another item that has to be mentally and visually processed. Driving safely requires focus and concentration, this label only adds a constant distraction while driving. Removing the label or placing it on the underside of the visor would improve visibility and safety.

- #11554255November 8, 2023LANE DEPARTURE: WARNING, VEHICLE SPEED CONTROL
My wife was involved in an accident with her Audi Q4 E-Tron on Monday, August 7, 2023 at approximately noon. She was driving into Boulder to pick up our granddaughter from an art camp in Table Mesa. On her way to Boulder, she noticed that the lane departure warning indicator light was not working which was odd because she had been using that since she purchased the car. She was also in Drive B (regenerative braking) mode. She drove into the parking area and was pulling into a parking spot very slowly. When she moved her foot off of the accelerator towards the brake, the car lunged forward at an incredible speed. She pushed on the brake pedal with all her might, but it did not stop her from crashing into a pole, another car and the store front of Weaver’s Dive Shop. This resulted in damages that were determined to be a total loss by the insurance company. We have researched the web to determine if this situation has happened to others. We were not surprised to discover that Unintended Sudden Acceleration is a known issue. However, this occurs very rarely and happens when the car’s computer gets confused by the signals from the battery, accelerator and inverter. Dr. Ronald Best has been researching Unintended Sudden Acceleration for over 10 years and has published a number of articles which document his findings and how Unintended Sudden Acceleration can occur due to what he refers to as a design flaw. We have saved & printed out documents which address this issue. I am attaching the most relevant article by Dr. Ronald Best. State Farm, our insurance company compensated us for the total loss of our vehicle. We were not made whole because the car was 10 months old. The State Farm report is attached. We submitted a request to Audi National Customer Service to research the car to determine if there were any issues and to have them compensate us if there were issues. Attached is their response followed by our response followed by their final response.

the horn is at an extremely low volume/pitch. Can barely hear it especially for other drivers inside their vehicle. Someone almost crashed into me and I pressed the horn but it was barely audible.

On September 22, 2023, I was parking the car in a parking garage and I had my foot on the brake. I was about to shift to reverse when my car suddenly accelerated on its own into a cement wall. My foot had remained on the brake the entire time. There have been other similar incidents with Audi Q4 eTrons as reported on this page: [XXX] - Unknown what component or system failed or malfunctioned as the vehicle is currently at a collision center for repairs. - If others had been near by, serious injury or death could have occurred. I experienced neck and back pain from the impact. - The problem has not been reproduced or confirmed by a dealer yet - The vehicle has not been inspected by the manufacturer, police, insurance or others yet - No warning lamps, messages or other symptoms appeared prior to the incident. As I was starting to make a right hand turn into a parking space in a parking garage, the car suddenly accelerated. I slammed on the brakes, but the stopping distance was not long enough. The car crashed into the steel cabling and concrete pole. The airbags deployed and the damage was significant enough to total the car. I sustained bruising and back muscle pain from the force of the impact. I only had the car for 11 days when this happened. I requested black box data for the car from the collision center and insurance company - neither would provide. I also reported the incident to my Audi dealer. Had there been a car in the parking spot next to me or directly in front of me, there would have been significant damage to other vehicles.

- #11530887July 7, 2023WHEELS, STEERING, LANE DEPARTURE: ASSIST
On April 14, 2023. I was driving west bound on Warner Road in Tempe. I bypassed the shopping enter and turned into the church parking lot - to drive through so that I could change the direction to go East on Warner Road. While driving through the parking lot - only doing about 10 miles per hour, the vehicle sensors "Adaptive Driving" feature - motioned the vehicle which case the right front to hit the base of the concrete lamp post. The parking lot surface consist of hairline cracks - which the sensors obviously picked up and caused the impact to the concrete base.

About 50 to 65% of the time when I put the car in reverse, the rear facing camera does not work. There is only black showing where the rear image should appear.

Upon starting the car after a long (100 mile) drive many error warnings occurred and the vehicle was very difficult to drive. We only attempted to back out of the driveway so it could be towed for repair. Audi has had the car for 4 months and has been unable to diagnose the problem and fix the car.

The rearview camera does not show when the vehicle is placed in reverse gear. We found that some Audi Q4 2022 was recalled #22V823000 but our car Q4 2022 was not. Now we got this issue. We believe that we should have be recalled and should be repaired free of charge. Thank you.
