The break actuator and booster pump failed while driving, causing the driver to experience an unexpected loss in breaking. This issue has been confirmed by an independent mechanic (issue code C1391 - Anormal leak of ACC press). Additionally, both the mechanic and dealer have confirmed this is a "common" issue with this vehicle. There are also a number of lights on including the red break light, ABS light, skid control light, etc. This issue represents a significant safely hazard for drivers, as it occurred unexpectedly leading to a loss of breaking power and appears to be common (there are wide spread reports online of other Prius owners experiencing similar issues AND we experience the same issue previously on a 2006 Toyota Highlander Hybrid). Requesting that NHTSA investigate this issue, and work with Toyota to issue a recall to address this defect and safety issue.

The Toyota Prius of this generation has a known defect in the brake booster pump assembly. About 2 weeks ago the dash lights came on for ABS, Traction control, general warning light. The brakes work but feel soft, and a motor sound pulses behind the steering column every 8-10 seconds. My mechanic pointed out it is a known problem where the ABS booster pump has a faulty valve that doesn’t allow proper pressure to build to engage ABS system. I cannot afford the Repair: $3890 for master cylinder and brake booster. My concern is that most people can’t afford this repair and without a functioning ABS system, people are put at risk from this generation of Prius. Toyota did offer a customer service program acknowledging the issue, but that program only helped if these issues occurred before 2022. With the known issue, why wasn’t a recall required? Toyota should be required to address this safety issue. How many of these Prius are on the road where customers are expected to perform a $4,000. Repair to keep up the safety standards. The car is available for inspection upon request. Thank you.

While driving on a highway, breaking became inefficient(very soft, failing to the floor) and car start showing "BREAK", ABS, VCS, Exclamation sign AFTER that. We had to drop the speed, exit highway (i95) and have car parked. It turned out that the booster pump has been working/turning on every 5 seconds and was barely able to create pressure in the breaking system. Now the booster pump, break actuator have to be replaced, breaks have to be bleed. Toyota knows about this being a repeat issue with its hybrids produced around 2013, and had been offering free repair for cars that are les than 10years old and have less than 150k miles, however, this is safety concern as breaks failure happens very unexpectedly, with NO prior warnings. Our car has only 98k miles, we are the original owners and it is just 10.5 years old which disqualifies it from Toyota's attempt to remedy this known safety concern. Toyota should recall all Prius V of this year, and replace booster pump and actuator BEFORE they fail on the road.

I was driving and all of a sudden the emergency brake light, the ABS brake light and a couple more turned on and I had no more brakes. I had just driven 25 miles with it and stopped at many red lights along the way with no issue and NO WARNING lights. I could have gotten into a major accident if it had happened anywhere else in my route. Fortunately, I was about a mile from my house and the hybrid motor helped slow the car down. I was unable to stop at any stop sign in my development and here again was very fortunate that there were no cars along the way. Once I got home, I got it towed to Toyota and was told it is an internal failure of the actuator. I contacted Toyota America and was just told that there was a campaign to repair this but that I had passed the date. My car has only been serviced at the same Toyota in my city since I got it. I religiously brought it every 5000 miles. I am shocked that they never mentioned this campaign to me and that they didn't initiate a repair during the campaign. This is a major issue that needs to be addressed and Toyota needs to be held accountable.

About three weeks ago, my daughter told me that several lights were on on her display. I checked and among them were the traction control and 2 other related to the brake system. I looked up possible causes on the Internet and it seemed the brake booster/master cylinder could be a culprit, but then I checked the fluid reservoir and the fluid was low, so I topped it off, drove the car, pumped the brakes a few times and all the lights went off. About a week later the lights were back on. I checked all the lines and saw not visible signs of leaks and decided to take it to a mechanic. I took it to a local mechanic and I was told that the brake master cylinder with brake booster failed and had to be replaced. Afterward the brakes were repaired I went to 2 local Toyota dealerships to check the price of the parts. The first dealership provided the prices without any discussion. At the second dealership, the employee who waited on me, after I provided her the VIN and told her that I need the price of the brake booster/master cylinder,m told me that there was a service notice on the booster/master cylinder because of a high rate of failure and that I would have to check with the service department before she could sell me the part. This is concerning, if Toyota knew/knows that the part has a high rate of failure, why wasn't a recall notice issued?

Brakes failed to stop vehicle for over almost ½ of a block. The driver, a young woman, was so terrified that she is refusing to drive the car even after the brakes have been repaired. It is my understanding that Toyota is aware of this problem and has issued directives for its dealers to make the necessary repairs. However, I was told that this was extended to 150,000 miles or 10 years from first in service date. I am including the invoice from Lipton Toyota for over $4,000 and I am respectfully requesting that Toyota makes this right and refunds this money. The failure of a braking system represents a critical life threatening situation of which no one can deniy.

I have a 2014 Toyota Prius V. I was driving down the highway at 55 m/h when my brakes became spongy all of a sudden and the ABS, traction and brake lights came on the dashboard. I was very scared and seeing that my brakes were failing I almost caused an accident that could have cost my life and my family's. I managed to get home safely and after scanning the system got the code C1391 - ABORMAL LEAK IN ACCUMULATOR. I did some research and this defect is so well known that Toyota launched a costumer program to replace the booster for free. I took it to a Toyota dealership and they wont't replace it though because the program expired a couple of months ago. This is an expensive and life-threatening part. The defect comes from the manufacturing facility, not from usage, so Toyota should be responsible for it.

Booster began working very frequently - every 10-15 seconds, computer scan tool indicated codes C1345 & C1368 done by independent service center. The dash display started indicating four warnings related to brakes: 2 different brake system warnings, ABS warning and SLIP indicator. Also, the brake action felt softer, the regenerative braking and ABS stopped working.

I am a US military member stationed in Germany. I bought my Toyota Prius V used, in 2020, knowing I'd be stationed in Europe and expressly because of Toyotas reliability and Prius's great mileage. This was our family's third Prius and a sixth Toyota vehicle. It's been almost exactly 2 years since my family moved to Germany, when one day I was driving through a small town, below the speed limit of 35 mph (50 km/h). Suddenly, while driving, the red BRAKE light, yellow ABS light, and red brake system warning lights came on all at once. The car's brake continued to work normally but I also started hearing a low volume, frequent, 1-sec long electronic buzzing sound coming from somewhere the front of the car, on the driver's side. It was audible every 3 seconds or so, even after the car was put in Park and turned off, then it went off after a couple minutes. I took the car to a local German Toyota leadership to have the problem diagnosed. The Service Department told me later that day the DTC1391 was identified, and the brake booster assembly + brake booster pump assembly needed to be replaced, at a total cost of this repair being over EUR 9,000 (around $9,900). The parts themselves were quoted to cost EUR 3,500 and EUR 4,000 each, and would have to be ordered from the US. I cannot afford to pay $10K to repair a 10-year old car. After some research, I discovered while Toyota never issued a recall, they knew about this brakes problem since 2018. I found several "Customer Support Program" notifications to Prius owners for this specific problem, including the one numbered 20TE07 which applied to 2012-2015 Prius's V. It told dealers to do this repair free of charge and extended the coverage for 10 years or 150,000 miles It also had info on repair cost reimbursement to customers. Hoping I could get my car fixed and file for reimbursement I called Toyota US and was told this CSP expired in Mar 2024 for my VIN and that there was nothing Toyota would be able to do.

After starting the car on 7/30/24, the Brake System Warning Light, ABS,Slip Indicator, and Master Warning Light all came on indicating an issue with the brake system. We were able to get it to the local dealership and when they ran diagnostics on it, it returned a code C1391, which confirmed the issue. This was addressed in 2020 by Toyota, but only for cars 10 years or 150,000 miles old. Our car is a 2022 with 105,000 miles on it, but we are outside of the year limit (car was 8 years old when the Customer Service Program was released).

2012 Toyota Prius brake actuator failure . While driving my Toyota Prius the braking system failed, this was on a large road and so posed a major danger to not only me, but the passengers in the vehicle and the vehicles around us. Thankfully nobody was hurt. After having the vehicle inspected by a Toyota dealership the problem was found to be the “Actuator/accumulator” and after speaking with the dealership it has come to be my understanding that this is a common and unavoidable problem with Toyota Prius models. There were no lights or warnings on in the vehicle until after the brakes gave out. I cannot understand why this has not been recalled when it is a manufacturing issue on Toyotas end that poses a significant danger on the roads, and am wondering what the reimbursement policy for this is as it came out to be quite expensive.

Four warning lights related to the braking system illuminate, followed by what seems to be the cessation of the ABS. After 5 minutes of driving, the lights went off and abs returned, although brakes are shaky. The abs seems to be a known faulty park and is covered under warranty but only up to 150,000 miles. When taken to a dealer, the repair shop recommended I call corporate. Citing the arbitrary mileage limit, the Toyota mechanic seemed convinced the corporate office would cover the cost, saying he’s seen it done on a case by case basis. This coverage should be made unilaterally for all car owners with this defect, not simply those willing to fight for it. Class action lawsuits over what seems to be a similar issue have been privately settled.

The contact owns a 2014 Toyota Prius V. The contact stated that the vehicle was previously involved in two car crashes, where the vehicle rear-ended other vehicles due to the brakes failure. The air bags did not deploy. A police report was filed after the second crash, but the information was not available. The contact stated that the other driver sustained unknown injuries during the second crash. The vehicle sustained damages and was repaired. There was no further information provided. Additionally, the contact stated while driving at various speeds and depressing the brake pedal, the brakes made an abnormal sound and the brake pedal was spongy. The contact stated that the vehicle hesitated to respond. The ABS warning light and several unknown warning lights were illuminated. The vehicle was taken to the dealer, where it was diagnosed that the brake booster had failed and needed to be replaced. The dealer informed the contact that it was a known failure; however, the repair could not be covered because the vehicle had a salvage title. The vehicle was not repaired. The manufacturer was notified of the failure and a case was opened. The failure mileage was approximately 120,000.

The Brake Master Cylinder and Brake Booster pump failed to maintain pressure. I don't have the replaced parts, my mechanic recycled them before I realised I was going to submit this. When the part completely failed, I had the ABS, traction control lights on. I couldn't trust that the car wouldn't have a problem stopped, and the brake pedal also became squishy in responsiveness. My own safety, my kids safety, and the safety of others was at risk and I immediately stopped driving it to get the repair done. The problem was confirmed by a Toyota dealership and this was a recall but since my car is older the recall had expired. I was unaware of this recall until the issue began. It was also confirmed by my personal mechanic to be the failure. The Dashboard lit up the ABS, TCS, Brake, (!) light, VSC, and the sound of the pump changed. Before all the lights came up I had noticed a air leaking sound coming from near the brake pedal at times.
