Hello, My name is [XXX] and I had the 2022 Tesla Model Y performance. I had an accident on [XXX]. Car just took control itself, we hit the tree. It was in a residential area, it happened so fast that I could not react to prevent it. After we hit the tree, the car caught fire, It was a nightmare for me, I had my best friend with me at the time of the accident in the front passenger seat. Both of us were unconscious, by the time I got awake, the car was in flames, my friend was still unconscious but then he got awake after I tried with the local neighbor to pull him out from the car. Unfortunately my friend passed away after an accident in 5 hours. I had autosteer assistance on my car as a subscription. Plus my car had a problem with the side cameras, from time to time it was giving me errors - Door Pillar error message. Cops were trying to get information from the car, but according to Tesla representative my car was out of network from June 30, 2022. And I never got any notifications that my car is out of network. I have a case against me VS State of New Jersey, the prosecutor's office charged me with vehicle manslaughter, at the time of accident i gave a statement to the police officer, in December 12, 2023 it was Nationwide recall on Tesla cars, and this recall it was exactly for the same problem what I described to the police officer at the time of accident, so my problem is, I can't retrieve any information from Tesla regarding what happened at the time of accident, DUE TO TESLA had my car out of network for 25 days prior. I can't prove my innocence because of that I'm facing serious charges right now. My complain is about my car being out of TESLA NETWORK FOR 25 DAYS, WITHOUT PROPER NOTIFICATION ON THE APP OR EMAIL, WHILE MY CAR WAS OPERATED EVERYDAY ON THE DAILY BASIS. WHEN USING CRUISE CONTROL (TESLA CALLS IT AUTOPILOT) EMERGENCY BRAKING IS APPLIED FOR NO KNOWN REASON. AT ONE LOCATION ON MY MORNING COMMUTE AT HIGHWAY SPEED IT OCCURS EVERY DAY AT THE SAME LOCATION, AT OTHER TIMES IT IS RANDOM. THIS INCREASES THE RISK OF A REAR END COLLISION WITH A FOLLOWING VEHICLE. I HAVE NOT REPORTED THIS TO TESLA. A FRIEND REPORTS THAT HIS 2023 MODEL Y ALSO BRAKES UNEXPECTEDLY AT THAT SAME LOCATION WHERE IT OCCURS FOR ME. IF DRIVING WITHOUT CRUISE ENGAGED THE PROBLEM DOES NOT OCCUR.

I have had multiple incidents of “phantom braking” when using traffic aware cruise control and auto steer. It usually happens on one lane state highways with opposite direction traffic, sometimes with turns or hills. The vehicle has slammed on the brakes without warning, sometimes the warning tone also sounds. Each time there was absolutely no danger, nothing abnormal. On a couple occasions there was traffic behind me, and it must have scared them. They probably thought it was a hard brake check. If I wasn’t able to quickly over ride the braking with the accelerator it very easily could have caused an accident. On two occasions my pregnant wife was in the car and it scared her very much. I am personally afraid to use it now, knowing it’s going to slam on the brakes is nerve wracking and UNSAFE. Tesla does not seem to care about addressing the issue, rather putting all eggs in the “full self driving” basket instead of improving this very obvious safety defect. From reading online forums it seems to affect just about everyone. I’m hoping their engineers address it and they release a software update correcting it, but as mentioned above it doesn’t seem to be on their list of priorities; the problem has been happening for several years now and has resulted in at least one known fatality. Hopefully the NHTSA can light a fire under them and have this scary safety defect addressed.

This morning when operating on FSD trail version, at tye stop sign I had to make left. This is at intersection of Rueslane and Old bridge Turnpike of East Brunswick, NJ. My car started to make a left when there was an incoming car from the left, and they do.not have to stop. Thank fully the car on the left applied instant brakes and avoided tye accident. FsD failed to make right judgement if left turn was possible or not. Fsd version is v12.3.6

With beta self driving engaged, I turned south on [XXX] intending to turn west on [XXX]. At [XXX], there are 2 right turn lanes. I approached the intersection turning from the left lane. Rather than turn right, the car continued straight. I was able to manually override the system and luckily brought the car under control; otherwise, I am sure I would have ended up in a very bad spot. Later the same day beta did not recognize a turn lane into my community but rather proceeded straight. I have since disabled the beta FSD. While driving on the highway got the warning on the control screen with red light warning stating "Pull over and stop the vehicle" and the car automatically started slowing down. Another warning popped up stating “Power Reduced - Exiting and re-entering vehicle may restore operation”. Has a yellow icon of a turtle. Felt very dangerous on a highway and immediately pulled over to the shoulder.

My car slammed on the brakes on the highway this morning at 75 mph. A box truck was following me but thankfully he didn't hit me. I was in what Tesla calls Traffic Aware Cruise Control (TACC) at the time and quickly brought the car out of TACC and hit the accelerator to get me back up to speed so I didn't pose a danger to other drivers on the highway. I dont know exactly how much my car slowed, but it was enough to throw me forward in my seat. There were no vehicles in front of me at the time. This was a classic example of "phantom braking", which is talked about a lot in the Tesla community. This is my 2nd report of phantom braking to the NHTSA.

Vehicle displayed error that service was required for front passenger safety restraint system fault in late September 2024, which persisted for several days. Customer immediately booked first available appointment booked with Tesla service center (3 weeks wait to mid October 2024). At dealership appointment, Tesla service advised it was unable to diagnose any problem because the error code had since cleared from the vehicle computer. Technician refused request for a manual inspection of the vehicle safety system. Technician refused to give paperwork or any form of documentation of the visit, insisting that the problem would be notated in Tesla's "internal service records" should Tesla need to review it at a later date. Subsequently customer received a "Service Appointment Cancelled" email notice from Tesla.

On Friday 10/11 at 10:45 a.m I was in traffic on Day Street in Moreno Valley, California. This was the third time I experienced a sudden unintended acceleration with my Tesla. I would have hit the car in front of me, but I was able to swerve into an island on my left to avoid the car. The car went over the curve and stopped. I am unsure what happened but after my prior experiences, I knew how to react. Had I not reacted swiftly, I would have hit the car in front of me. There were no warning messages. I have an appointment with Tesla on Wed 10/26. When this happened the first time, they claimed there was nothing wrong with the car, but I think they are not looking hard enough. I am aware of a voltage problem that other Tesla owners have experienced that can lead to sudden unintended acceleration.

My Tesla still has the phantom braking problem. It is repeatable and consistent. I can tell when it will occur on a route that I drive regularly. Day or night does not seem to alter the occurrence. There is no indication on the touch screen when this occurs. Traffic volume does not matter. It always occurs at the same places so it should be relatively easy to fix if someone would just monitor my computer when it occurs. I regard this as a dangerous problem on the freeway because my car suddenly slows and if the person behind me is not paying attention, I am going to be rear ended. Such a serious fault for a vehicle that performs so well otherwise. Since Tesla does not have a service center in my town no one has offered to look at the problem but since it has been reported so often, I'm not sure they would. I would be happy to demonstrate the problem to a technical person who might be able to diagnose it.

Rear passenger side seatbelt stopped working. It clicks when it is inserted and immediately becomes loose. It does not hold up and creates a serious safety issue. I have seen many people reported this issue online in Model Y Teslas. Nearest appointment Tesla provides is 2 weeks away and the seat cannot be used due to non working seat belt. I have never seen a seal belt malfunctioning in my entire life. It has happened many Tesla owners and these seat belts are not safe.

Got a new battery for the car due to an accident I had back in Feb 2024. The battery lasted me up to August 2024 before I received an error saying that my car won’t charge any more and it’s at maximum charge. I took my car in and the Tesla service center explained there is no physical damage, and that it’s a mechanical issue

In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.

We have had numerous incidents of sudden and unexpected braking. Earlier this month while rounding a curve with adaptive cruise control enabled, the car aggressively braked when it saw an oncoming semi safely in the other lane. It was good there were no cars behind us or no doubt they would’ve rear-ended us. Last night - like so many other times the past year or so - I was pulling into a parking spot with a bush in front and, only a few feet in, the car slammed the brakes on even engaging ABS. Usually the warning system just makes a sound or gently applies this brakes but this was jarring. Everyone in the car lurched forward. The date below is from the incident related to the semi. That could have been very bad. My Subaru Outback uses sonar and I’ve never had these sorts of problems.

The contact owns a 2021 Tesla Model Y. The contact stated that his wife was driving at 5 -10 MPH and traffic ahead was slowing down, the vehicle in front stopped and the driver depressed the brake pedal however the vehicle then unintendedly accelerated. As a result, the contact crashed into the rear of the vehicle in front. No warning lights were illuminated and no air bags were deployed. No police report was filed due to no injuries being sustained and the insurance company would be handling the incident. The vehicle was towed to a body center where it was diagnosed that the front air bag sensors were fractured in half and failed to activate the air bags but did activate the seat belts during the impact. The vehicle was not repaired, and the repair decision from the insurance company was pending. A dealer was not contacted. The manufacturer was informed of the failure via the app however the contact was still waiting for a return call to speak with a live agent. The failure mileage was 18,680.

The contact owns a 2024 Tesla Model Y. The contact stated that while at an undisclosed speed, the vehicle had accelerated independently. No warning lights were illuminated. On another occasion, while exiting a parking lot the vehicle unintendedly accelerated causing it to crash into a parked vehicle. The vehicle came to a stop partially sideways after it drove over the parked vehicle. The fire department arrived on the scene and safely removed the contact from the vehicle. It was unknown if a police or fire department report was filed. The contact sustained minor bruises on the right arm and sought medical attention a day later. The vehicle was towed to an insurance lot where it remained for further inspection. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was unknown.
