ELECTRICAL SYSTEM:ADAS:AUTONOMOUS/SELF DRIVING:SOFTWARE
The contact owns a 2022 Tesla Model Y. The contact stated that while driving at various speeds with the Full Self-Driving (FSD) activated, the destination was entered, and the system announced the direction steps when suddenly the Full Self-Driving (FSD) disengaged independently causing the driver to be required to take control of the vehicle. During the independent disengagement of the Full Self-Driving (FSD), the vehicle abruptly decelerated. The contact was concerned that the failure could result in a crash if a rear vehicle was tailgating. In addition, the contact stated that the Full Self-Driving (FSD) independently disengaging occurred during excessive sun exposure weather. The Tesla Service Advisor was informed of the failure and advised the contact that moisture entered the camera and the windshield causing the failure and it was a known issue but only an interim repair was currently available. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 35,000.

On a separate occasion, the contact stated that the Full Self-Driving (FSD) was engaged and driving at various speeds, and the vehicle independently maneuvered into oncoming traffic on a 4-lane road causing the contact to have to take over control of the vehicle. A crash was avoided; however, the contact was concerned that if there had been a median barrier it could have resulted in a crash. In addition, the contact stated there was a second occasion where the vehicle was in Full Self-Driving (FSD) and veered into oncoming traffic, and manual control of the vehicle was required. The failure was recorded via the Tesla App and the data was entered via AI. A dealer was contacted, and a service appointment was scheduled for a diagnostic test. The Tesla Service Advisor informed the contact that moisture entered the camera and the windshield causing the failure and it was a known issue, but only an interim repair was currently available. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

While driving a front passenger seat occupant classification system (OCS) error appeared. Made appointment February 7, 2025, Tesla replaced front row seat harness assembly as well as the front row occupancy sensor. Invoice # 3000S0012299476 states they did a retrofit service and proceeded with the required update and replaced sensor with the latest version.

STEERING
While driving heard a noise from front end. As I turned right the steering wheel turned left in opposite direction. I heard something fall and found a bolt that fell off the car control arm. I managed to get the car back to my garage and called Tesla service. They say it needs an alignment and are going to charge me for this repair. A bolt should not be falling off the vehicle.

Several times on residential speed control the system does not recognize yellow flashing school speed reduction signs. Never slows or breaks. The speed of 25 mph does not register on the screed (other speed signs do). I told tesla service center when other services were being done. They said they were aware. I have seen no improvement over the last several months

The contact was an attorney calling on behalf of the owner of a 2025 Tesla Model Y. The contact stated that the owner was driving on a neighborhood street at an unknown speed with Autopilot engaged when vehicle control was lost. As a result, the vehicle crashed into a traffic light pole where it came to a stop and caught on fire. The owner was unable to exit the vehicle and was pronounced deceased on the scene. The origin of the fire was the lithium battery. The location of the fire was under the front end of the engine compartment where the battery was stored. The fire was extinguished by the fire department. A police report was filed. The vehicle was towed currently in the possession of the Police Department pending the cause of the crash. The vehicle was destroyed. The light pole was damaged. It was unknown if the air bags were deployed. The manufacturer was not notified of the failure. The failure mileage was unknown.

Subject: Urgent Safety Concern – Steering Component Failure in 2023 Tesla Model Y Dear Tesla Customer Support / NHTSA Safety Division, I am writing to formally report a serious safety issue involving a critical steering component failure in my 2023 Tesla Model Y (VIN: [XXX] 38,917 miles). The failure led to a loss of control and resulted in a collision. On [XXX], my wife, [XXX] , was driving the vehicle on our residential street, shortly after leaving our driveway. At a low speed of approximately 8-10 mph, she suddenly lost steering control as the car rapidly veered to the right. Despite attempts to steer, the vehicle was unresponsive, leading to a collision with a parked car. Damage Summary: •Our Tesla Model Y: Damage to front bumper, headlights, right wheel cover and right fender. •Parked Vehicle: Deep scratches and dents on the rear driver-side bumper and passenger door. Upon inspecting the vehicle after the incident, it became apparent that a suspension or steering component (likely the lower control arm) had failed (broken). After researching online, I discovered that multiple Tesla Model Y owners have reported similar sudden failures of the same part, indicating a potential manufacturing defect. Request for Immediate Action: 1.Tesla Inspection & Full Repair at No Cost – Since my vehicle is under warranty and this appears to be a manufacturing defect, I request a full investigation and repair at Tesla’s expense. 2.NHTSA Investigation into a Potential Recall – Given that similar failures have been reported by other Tesla owners, I urge NHTSA to review this issue for a potential recall before more accidents occur. 3.Tesla’s Official Response – I would like Tesla to confirm whether this component has been investigated for defects in other vehicles and what steps are being taken to prevent other accidents. My car is equipped with autopilot feature which I am unable to use due to safety score system designed by Tesla which is used for calculating insurance premiums. This safety score system doesn’t count the miles driven on autopilot and only counts the miles driven without autopilot to decide safety score which impacts insurance premium. This policy forces consumers to drive car without autopilot to increase number of miles driven to compensate for any mistakes made during driving without autopilot. This not only compromises safety of the vehicle and passengers but also deprives me to use features of my car that I paid for. Since autopilot is a supervised driving and requires driver active engagement the miles driven using autopilot should also be counted to decide safety score. Dealership are unable to help with any issues related to safety score system and there is no customer support available to address customer concerns.

I was driving on a three-lane highway (I-495) using the adaptive cruise control (autosteer) when I suddenly noticed the car to my right dangerously veering toward my vehicle. In an attempt to change lanes to the left, my car wildly overcorrected to the left, nearly sending me into the highway median, followed by another overcorrection to the right. The steering wheel felt almost uncontrollable. Luckily no collision.

When using FSD, I've noticed that on occasion at a red light, the car will start moving, albeit very slowly, a fraction of a second before the light turns green. I've assumed the car was watching the traffic lights from the other direction using its cameras. So on the [XXX] when this happened, and the car started moving I let it go thinking the light was about to turn green. But I was shocked that the car drove through the red light. Admittedly there were red lights in all directions. I know I'm supposed to be keeping control but it was so shocking that by the time I reacted the car was already in the intersection so I let it go. I was trying to reverse the car from my Garage. I turned on the Engine and put the vehicle in Reverse Gear. The vehicle just zoomed out of the driveway leaving me without the ability to control it and hit a tree on a neighbor's house and stopped. Then I tried to bring the Car into the Garage by engaging the Drive Gear. Again, the vehicle zoomed into the Garage, hit a few objects in the garage and stopped. At that point I just turned off power and walked out. I took pictures of the damaged parts. At that time, I only noticed damage to the Rear on the Driver Side and the front on the Driver Side. When a tow-truck came a couple of days later, I noticed damage on the Front on the Passenger side also. When the incident happened , the street was empty and luckily no one (including me) was hurt n

The contact called concerning the owner of a 2024 Tesla Model Y. The contact stated while idle at a red light in her vehicle, the 2024 Tesla Model Y rear-ended her vehicle. When speaking with the owner of the vehicle it was informed that the Tesla was on autopilot causing the crash. The owner stated that they could not feel when the impact occurred. No air bags deployed. The contact's vehicle was damaged near the license. The contact stated she sustained a soar neck but medical attention was not sought. The owner of the Tesla did not sustain any injuries that were known. A police report was not filed. The contact was unsure if the dealer was contacted or if the vehicle was repaired. The contact was unsure if the manufacturer was informed. The failure mileage was unknown.

SUSPENSION
My wife was driving her 2023 Model Y with our [XXX] son in it, suddenly she heard a loud sound back to back twice and the car stopped, she couldn’t drive or move the car, she stepped out and saw 2 large bolts on the ground near the front passenger tire and the tired turned inwards and the tire would not turn upon steering and would not move at all. We called Tesla Roadside assistance. The tow technician reviewed the situation and said this should not have happened and it is a major issue. If this happened on a freeway it could have been a disaster. He also told us that he has to try to get the bolts back in after removing and breaking some of the adjacent plastic cover on the bottom of the car in order to safely tow the vehicle without additional damage. The car was towed after 2 hrs of breaking adjacent plastic next to where the bolts were supposed to be and trying to get bolts back on to straighten the wheel. Truly shaken, if this happened on a a freeway or when car was fast, it could have been a devastating accident involving life of my child and wife. We are still trying to fathom what happened and why. We will talk to the Tesla service center tomorrow. We found at least one other post on forums re: similar issue with Tesla. VISIBILITY:REARVIEW MIRRORS/DEVICES:EXTERIOR
The model Y has a software feature which integrates with MyQ Garage Door opener which opens the garage door whenever the car is nearby. This feature also has a selectable option to fold mirrors automatically. It is noticed that the mirrors fold even when the vehicle is in motion or not in park. This is especially dangerous when backing out into the garage. I almost had 2 near misses when the mirrors which I was using to reverse the car suddenly folded in the middle of the maneuver. Ideally the mirror should only fold if the car is in park or at least not traveling in reverse

Collision while using Summon feature on a Tesla. I was parked, rear end in, in a parking stall at work. A minivan was parked, front end in, in the stall adjacent to me. The two cars were facing opposite directions with the drivers side of each car next to each other. As I approached my car, from a distance of about 100ft, I used the Tesla app to summon my car to me. I pressed and held down the "Come to Me" button and my car lights went on and the car began moving forward then made a sudden, unexpected sharp left turn before pulling out of the stall. The turn was too sharp and a collision was imminent. I let the button go on the app, but it was too late and my drivers side back door panel scraped the rear drivers side bumper of the minivan next to me. I got into the car, manually backed up and surveyed the damage. I thought it was minor, but I reported it to the minivan owner. Due to an estimate of damage in the several thousand dollar range, I filed a claim with my insurance company. They repaired the damage to the minivan. I deferred any repair to my own car because I thought it was minimal. No injuries occurred as the result of this collision. I did not mention the use of the Summon feature when I reported this collision to my insurance company. I simply mentioned that I hit the car next to me pulling out of a parking stall and that the fault was mine.

SERVICE BRAKES, UNKNOWN OR OTHER
After a snowstorm, I shoveled my driveway in front of the car which was facing toward the road (the driveway is on an incline, not steep), cleared the snow off the car and then pulled the car forward/put it in park so that I could shovel the rest of the snow. It took me and my 15 year old son 10 or so minutes to clear the snow behind the car. Then I started to salt the driveway behind the car and a snowplow driving by started honking. I looked up and the car was sliding down the driveway towards the road. Luckily it slowed and stopped, only slightly into the road and did not hit the snowplow, or anything else and I was able to get in and move it. I have a video clip of it which I am unable to upload here - the front tires were rotating while the back tires were locked in place.
