Full Self Driving caused my Tesla to enter an intersection while the light was red. The FSD stopped at the red light on Monday Dec. 22. 2025 at 9:37 am. After a few moments is began to drive from the correct stop and enter the intersection while the traffic light was red. I had to manually take control to prevent it from continuing thru.

On Sunday evening (12/21) at approximately 6:30 pm, I enabled Self‑Driving while my car was parked in my garage with the garage door open. The vehicle was positioned facing forward. Immediately after activating Self‑Driving, the car began reversing rapidly in an apparent attempt to exit the garage. During this unexpected movement, the passenger‑side mirror struck the garage door trim and cracked. I quickly applied the brakes and disengaged Self‑Driving to stop the vehicle. This was a frightening incident, and I was unable to submit an FSD bug report because the reporting prompt disappeared within seconds.

While operating my Tesla Model Y in a commercial parking lot at a T-intersection controlled by a stop sign, the vehicle unexpectedly and automatically accelerated without driver intent. The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. Despite clear manual brake application, the vehicle did not decelerate as expected and continued to move forward. I also attempted to stop the vehicle by pressing the Park button, but this action did not stop the vehicle. The vehicle continued uncontrollably for approximately 150+ yards, traveling over curbs, striking a fence, and ultimately colliding with the garage structure of a residential home. Throughout this sequence, the vehicle remained unresponsive to braking and stop inputs. Vehicle data later confirmed that brake pedal application was detected prior to impact, yet braking response was insufficient to prevent continued motion. At the time of the event, the vehicle recorded system fault states, and collision avoidance systems did not activate. Despite significant structural impact and property damage, airbags did not deploy. The incident raises serious safety concerns regarding: •Brake-by-wire system execution during fault conditions •Pedal input arbitration between accelerator and brake •Failure of collision avoidance systems to intervene •Failure of restraint systems to deploy during a severe impact •Vehicle behavior when control systems are in a fault state This event occurred without warning and despite appropriate driver response. Based on vehicle data and system behavior, this appears to be a system-level failure rather than driver error.

All in the same day: 1) Odd sound from behind the pedal area of driver side front quarter (sounded like a styrofoam cooler top rubbing against the cooler is the best way to describe - faint though). 2)BANG when in a parking garage turning full right but still able to control without difficulty. Made it home without incident. 3)Later that day, just after getting off of I4 in Orlando I pulled into grocery parking lot and parked. When I returned to my car I again turned right and a loud BANG and the car jerked to a sudden halt. I was able to back into a parking spot but there were 2 large screws in the driving land and the left wheel was toed slightly to the left. Ultimately it was detached when we tried to tow it we had to remove tire and put on a device to allow us to move it. Per Tesla “I have updated that estimate! The vehicle has very heavy damage, and this is the start. Once we replace these components their (sic) could be more damage, and it also could potentially need to go to a body shop!” Thankfully this didn’t happen at speed with my daughter in the car on I4! This is unsafe and a simple internet search revealed it is not an uncommon parts failure at this mileage! This is dangerous. All of this at just 54,000 miles. (4k out of warranty)

Incident Summary: Car Computer (HW3) suffered internal hardware failure on 12/17/25 following software update 2025.44.3. While the media screen and rearview camera functioned, the Autopilot Board (APB) was "MIA." Tesla Randolph (Invoice #3000S0015357298) confirmed the internal failure and replaced the unit. Safety Risk: Failure disabled safety-critical ADAS: Automatic Emergency Braking (AEB) and Forward Collision Warning. Driving visualizations were severely compromised; the vehicle failed to detect pedestrians or lane markings while in motion, creating an immediate collision risk. The car’s active safety "intelligence" was essentially blind despite the camera feed being visible on the screen. Inspection & Contradictions: Tesla Service confirmed the "internal failure" (12/19-12/23). Note: The Service Manager verbally claimed "no codes," but the official invoice explicitly states technicians "found multiple alerts present." Part was retained by Tesla. Warning Symptoms: Failure followed a software reboot loop. Symptoms (MIA board/power cycle failure) match Recall 25V-002. Vehicle had only 36,289 miles. Reporting as potential HW3 defect expansion.

The contact owns a 2022 Tesla Model Y. The contact stated while his wife was driving 40-50 MPH in the evening with Self-Driving mode activated, the vehicle failed to decelerate while driving onto the exit lane and crashed into the rear of another vehicle. The contact was occupying the front passenger's seat during the incident. The front driver's side and passenger's side air bags deployed. There was no reported injury. The vehicle was towed to the residence. The vehicle was later towed to a Tesla autobody repair shop to be repaired. The vehicle was then towed to the autobody repair shop recommended by the insurance company. The vehicle was later totaled by the insurance company due to the cost of the air bags replacement. A police report was filed. The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

Vehicle: 2022 Tesla Model Y VIN: [XXX] Mileage: 85,000 Complaint Description (Copy & Paste): I am filing this complaint due to serious and ongoing safety-related defects with my 2022 Tesla Model Y that remain unresolved despite multiple service visits. The vehicle experiences shuddering and vibration during regenerative braking, especially when driving downhill. In addition, the braking behavior is unpredictable, which compromises vehicle control and creates a risk of loss of control while driving. These issues occur during normal driving conditions and present a significant safety hazard to myself, my passengers, and other road users. I have taken the vehicle to Tesla for service three separate times for this issue. Despite these visits, the Tesla service department has stated they are unable to fix the problem, and the safety concern remains unresolved. I’m being having this issues since I bought the car new. When in self driving mode, which activates the adaptive cruise control, it is not possible to set the following distance. The following distance automatically selected by Tesla self driving is much too close to the vehicle in front of me. Tesla has removed the ability to set the following distance. It follows at approximately 2 seconds behind the car in front of me, regardless of my vehicle speed .... at 80 MPH 2 seconds is not enough time for a driver to react. Following distance should be controllable by the driver. Taking away this ability deprives a driver of driving within their own limitations.

Tesla self driving detects and sets a speed limit and displays it as a speed limit icon. I used self driving to exit my driveway, which turns onto a dirt road with a speed limit of 15 MPH. Near the bottom of the driveway self driving suddenly reset the speed limit from 10 MPH to 55 MPH. The car then accelerated abruptly shortly before the turn onto the dirt road. There is no mechanism for overriding the speed limit the Tesla self driving automatically selects. It is clear that the speed of the vehicle is adjusted according to the speed limit that Tesla self driving selects. Before I could regain control of the vehicle I was almost into the field opposite the driveway. The inability to re-set the automatically selected speed limit is dangerous. The speed limit has spontaneously reset to 55 MPH on two other occasions, and the vehicle spontaneously speeded up.

The right rear taillight and turn signal on my Tesla Model Y became dim and intermittently flicker, and the turn signal does not function properly. This issue began immediately after a Tesla software update. The malfunction significantly reduces rear visibility and signaling to other drivers, especially at night, creating a safety risk. I contacted Tesla Service and provided photos/videos. Tesla have scheduled a repair which is 1 week later but declined to confirm whether the vehicle is safe or legal to drive with this lighting malfunction. Tesla also declined to provide a loaner vehicle or alternate transportation and advised me to use my own judgment. Rear lighting and turn signals are safety-critical components required by law. Operating the vehicle in this condition poses a risk of rear-end collision or failure to signal lane changes or turns. I am filing this complaint due to safety concerns and Tesla’s inability to provide written guidance confirming safe operation until repair.

The contact owns a 2024 Tesla Model Y. The contact stated that while driving at various speeds, the TPMS displayed that the PSI of the tires was low. Additionally, a notification advising that the vehicle was experiencing low voltage and drainage and would shut down was displayed. The contact called towing services. The contact stated that the towing agent who assisted in towing the vehicle informed the contact that the tires were losing air, and the front passenger's side tire or wheel was defective. The vehicle was towed to a Tesla Service Center, where the power converter system(PCS) was corrected. Additionally, the technician informed the contact that the passenger's side tire was leaking air due to a rim defect, but the vehicle was not repaired, and the contact was advised to pick up the vehicle. The contact stated that the vehicle could not be driven at 20 MPH due to the failure. The contact had not retrieved the vehicle from the Service Center. The contact related the failure to NHTSA Campaign Number: 24V935000 (Tires), but the Tesla Service Center informed the contact that the recall repair was completed. The contact stated that NHTSA Campaign Numbers: 24V554000 (Latches/Locks/Linkages, Electrical System) and 24V935000 (Tires) were marked as complete on the Manufacturer's website; however, the NHTSA VIN tool displayed that the recalls were incomplete. Additionally, the contact stated that the driver's seat was not adjustable. The contact stated that while attempting to move the seat higher and forward, the seat turned inward towards the right. The vehicle was not repaired. The failure mileage was approximately 100,000.

I am reporting a significant safety defect on my 2026 Tesla Model Y (delivered in October 2025). The vehicle is missing the lighted interior emergency release button inside the front trunk (frunk). Upon inspection, not only is the button missing (replaced by a blank plastic panel), but the necessary wiring harness to support a release mechanism is also absent. This appears to be a deliberate design change that violates FMVSS No. 401, which requires an interior release mechanism to prevent entrapment. A child or small adult could potentially be trapped in this compartment with no way to exit from the inside, creating a life-threatening entrapment risk.

Incident summary (Sanford, FL – Appaloosa Court – Dec 2, 2025): In the early morning on Tuesday, December 2, 2025 (around 5:00–5:15 a.m.), a fire occurred at a residence located on [XXX] . A Tesla located in the garage was reported as being involved, and the fire spread to the garage that totaled the house The vehicle was reportedly charging in the garage when smoke/fire was noticed. Emergency responders (Sanford Fire Department and Sanford Police) responded to the scene. The incident resulted in property damage and at least one injury reported as non-life-threatening (per local reporting). Fire forensics stated that the fire started and caused by the TESLA around the charging plug area by the rear left passenger door.
