At approximately 5:50 PM on Feb 12, 2026, a collision occurred while the vehicle was under the control of "Full Self-Driving (Supervised)." While parked in a commercial lot, I engaged FSD to initiate a route home. A stationary semi-truck was positioned behind my vehicle. Upon engagement, the FSD system failed to detect the presence of the stationary truck. The vehicle initiated a rearward/lateral maneuver at a speed of 1 mph, directly striking the corner of the semi-truck. Safety Failure Details: The collision resulted in significant damage to the right rear quarter panel and associated sensors of my Tesla Model Y. Dashcam footage and system telemetry confirm that FSD was active (Supervised mode) during the entire duration of the maneuver. The system's occupancy network and vision suite failed to identify a large, stationary object within its immediate path of travel at a crawl speed. Request for Investigation: I am reporting a critical failure of the FSD software’s object detection and path planning capabilities. The system commanded a movement into a clear obstruction without issuing any takeover alerts or applying emergency braking, despite the low speed and high visibility of the obstacle.

Front driver side lower control arm bolts came off without external forces. Both bolts fell off with no damage to the threads. Tesla service stated that upon inspection of their technician it is determined that the front left lower link bolt was either not properly seated or missing which caused a defective front lower link bushing. (I included a screenshot of their finding). The damage was caused by this defective part leading to the car not being unsafe and inoperable due to no steering and no drive as well as wheel literally coming off the car while in motion. Luckily I was simply backing into my driveway when this failure happened narrowly avoiding a potential life threatening accident. I have researched that this was a known issue for my exact make and model just months before my car was purchased in march 2022. I have attached the document "Part 573 Safety Recall Report 21V-835. Unfortunately, Tesla refuses to admit the defect when it was time to pay for the repairs despite clearly admitting it on their initial inspection, leaving me with the repair bill.

Vehicle Information 2025 Tesla Model Y Approx. 30,000 miles Purchased new ~11 months ago Incident Description While driving at highway speed, the front trunk (frunk/hood) of my 2025 Tesla Model Y unexpectedly opened. The hood immediately flew upward, flipped backward over the windshield, and shattered the windshield. The frunk had not been opened in approximately two weeks prior to the incident. The vehicle had not undergone recent service or repair involving the hood or latch assembly. There were no prior warnings or alerts indicating that the frunk was unlatched or not latched properly before driving. The hood appeared fully closed prior to the trip. However, after the incident once we pulled over, a bolt was completely out of place, as shown in my video. Even if the primary latch was faulty, the secondary latch / catch should have prevented the hood from opening all the way and slamming the windshield. This failure to notify improper latching created an immediate and total obstruction of forward visibility at highway speed and posed a serious crash risk. The vehicle has approximately 30,000 miles and is less than one year old. Given Tesla’s prior recall related to hood latch detection issues, I am concerned that either: •The latch system failed mechanically, and/or •The vehicle failed to properly detect and warn of an unlatched hood. This is a significant safety event and could result in serious injury or death if it occurs in traffic.

On February 11, 2026, the 2025 Tesla Model Y "Vision AutoPark" system initiated an uncommanded high-speed reverse acceleration into a stationary yellow concrete bollard in a parking lot. The vehicle's vision-only parking assist hardware failed to detect a clearly visible, fixed obstruction and displayed a clear path on the system interface immediately before impact. No warning lamps, chimes, messages, or other alerts preceded the failure. The system provided zero indication of the imminent collision. The vehicle closed the distance to the bollard in under one second, exceeding human reaction time and making manual braking intervention impossible before impact. This failure mode — autonomous acceleration toward a stationary object without detection or warning — creates a severe crush hazard for pedestrians and property. The vehicle and its onboard data logs are available for inspection upon request. High-definition video evidence of the failure has been preserved showing the system ignoring the visible obstacle. The problem has not been confirmed by a dealer. The manufacturer's service center refused to inspect the vehicle or review data logs on the date of the incident despite an immediate in-person request. The vehicle has not yet been inspected by insurance, police, or the manufacturer. The failed component is the Tesla Vision-only AutoPark system, including its camera-based object detection and autonomous vehicle control software.

I am reporting a safety incident involving my Tesla while Autopilot/Full Self-Driving was engaged. On February 11, 2025 at approximately 9:22 am, on Waccamaw Medical Park Ct Conway, SC 29526, I was driving on a normal city street with the system actively controlling steering and speed. Without any warning or alert, the vehicle suddenly steered to the right toward the curb and struck it. I was very attentive, but the steering movement happened too quickly for me to safely prevent the impact. No forward collision warning, lane departure alert, or disengagement occurred prior to the incident. The impact caused damage to the wheel and tire (see attached photo). This behavior appears to be an incorrect steering decision by the automated driving system after misinterpreting the roadway edge or lane boundary. I am requesting review of the vehicle logs and investigation of the system behavior during this event.

Received a notice that steering assist may be reduced. Other model year Ys have been recalled due to the same issue. It doesn’t make sense that this should fail in a vehicle that is 5 years old with no accidents and low mileage. This is a safety issue due to manufacturing defect and should be covered under warranty.

On Saturday morning January 3, 2026 I was driving with my wife, daughter and dog. There was light rain at the time and I had my adaptive cruise set at approximately 72 mph. I was driving our 2024 Tesla Y. At approximately 10:21am the rear of car began to slide right as we entered gradual left curve, possibly because we hit water on road. I tried to slightly correct right and when I did that the steering wheel aggressively corrected back left which put us into a slide. Within a second we hit a tree on the passenger side. The impact was severe and on the passenger side door where my wife was sitting. At that time I theorize that my dog was ejected out of the back window and thrown across the freeway. We then rolled in the other direction (I dont know how many times) down a hill and settled upside down. I immediately smelled smoke and began to try get out of my seatbelt but I couldn’t. I carry a knife and I was able to cut myself out. I then tried to open door and could not so I began punching the glass and eventually got it moving and pushed on it and got it open. I slid myself out, turned around and saw car was on fire. I pulled my daughter out and then began extracting my wife. She was unconscious and had obvious orthopedic injuries. I am 100% confident that had I been driving my other vehicle (non Tesla) that this accident would have NOT occurred. The Tesla and its steering system caused this accident. On top of that, the batteries immediately caught fire, my seatbelt release button did not work, and the electric door button did not work. Had I not been carrying a knife and strong enough to punch door open this story would have ended up differently. The car completely burned. As far as I know, this was the first time this exact issue presented itself. However, when looking online its seems this issue is fairly common with Teslas in wet weather.

It was snowing and I was driving slow. About 8:40am, Feb 6th, 2026, I got a "One pedal driving disabled" alert just for a moment, then tried to stop before an intersection because a signal was turning to red. However, break didn't work and I had to enter the intersection under a red signal. Fortunately, there was no car in front of me, also a car coming from the side didn't notice signal changed. No accident, but very very scary. On Dec 6th, I also got an alert "One pedal driving disabled" and even after I released an accelerator pedal, speed didn't slow down. I reached Tesla on Feb 6th, but the service scheduled 4 weeks later. It's not safe to drive at all, thus, I will try to have them to check my vehicle asap.

The emergency mechanical levers for BOTH of my front doors do not work at all. I pull them while pushing the door and they do not open the car. If the power dies in the car, there is no way for me or my passenger to escape the car. I reported this to Tesla and they are not able to immediately investigate it or provide me replacement vehicle.

We live on a road with a steep embankment. My wife backed across our driveway and when she put the car from reverse to drive it accelerated uncontrollably. She hit our mailbox, then a power pole across the street, a driveway across the street then over the embankment where she hit a retainer wall another wooden railing and finally came to rest against our neighbors house. If she wouldn’t have caught the edge of the house she would have went down approximately 150 foot embankment. Not only the car accelerated on its own it is supposed to automatically brake, it didn’t and the airbags never deployed. This all this happened right after it went through an automatic update. The car was totaled, with my wife getting a concussion and a lot of muscle injuries. There was no warning lights prior to event and we reported it to Tesla and all they did was send camera images which we had. The car is now in possession of Wawanesa Insurance because it was a total loss

Went for a tire rotation. Service tech noticed the following- THE PASSENGER REAR CONTROL ARM HAD A BOLT THAT WAS BACKING OUT OF IT & THE NUT WAS MISSING. THE BOLT HAD RUBBED THROUGH THE WHEEL WEIGHT ON THE WHEEL THAT WAS ORIGINALLY ON THE RIGHT REAR. Photo is included of the rear control arm bolts for both sides of the car. Only the right rear had a problem.

On [XXX], after placing my young child in my Tesla and closing the door, the 12V battery failed suddenly and the vehicle became inaccessible. I was unable to unlock or enter the car. I did not receive any warning or alert that the 12V battery was about to fail. The vehicle was in my garage, and the window automatically cracked, which allowed me to monitor my child while I sought help. I called Tesla Roadside Assistance at 8:59 AM. During the call, the representative reassured me that help was being dispatched and at times described the service provider as “on the way” (timing details such as “within ~45 minutes” and the length of the initial call are my best estimates). After continued waiting with my child confined inside the vehicle, I contacted emergency services due to the safety risk. I called 911 at 10:03 AM. I later had the 12V battery replaced at my own expense. This incident involved a sudden electrical failure without warning that resulted in a child being trapped inside a disabled vehicle, and I am also concerned about the accuracy and effectiveness of roadside assistance dispatch communications during the incident. Before this, I noticed symptoms consistent with reported Model Y issues: rattling, clunking/popping noises, particularly when driving over bumps or turning, and unstable steering. This appears to be a manufacturing defect, similar to previous recalls for under-torqued bolts.
