On Saturday, August 22, 2026, while traveling on the highway with my two toddler-aged children in the vehicle, the temperature gauge entered the red zone and the vehicle automatically reduced power due to overheating. This forced me to navigate a power-reduced vehicle on a highway with minor children — a serious safety hazard. This incident occurred just five days after my vehicle was in the Sewell Lexus service department on August 17, 2026 for its 10,000-mile service and a warranty repair to the rear evaporator case (a known design defect). The proximity of this safety incident to that service visit raises significant concerns about whether the August 17 service contributed to or caused the overheating event.

When starting from a stop, putting foot on the gas pedal the car won’t move. Hesitates for a longtime. It has happened numerous times since purchased new. The manufacturer stated that engineering is looking into it, with no definitive answers. No warning lights. It just won’t go, at times which is a safety concerning especially when crossing with oncoming traffic. Unresponsive.

Strong smell of gas coming from around the car and near rear end of car. Usually after a drive. The smell does not happen after every drive. I brought up the smell to my husband a number of times within the year…and finally took to JD Auto in Arizona in June but the gas smell could not be replicated and no source or defect could be visually seen. No recall for this has been reported by Lexus. dealership. No check engine light has been recorded. Worried that there is some internal issue that could cause huge problems. I do have children who have also reported smelling the gas in the back seat.

We have had 2 windshields crack on the Lexus TX350, with very low impact to the windshield by very small road debris. Both windshields require(d) replacement due to long migrating cracks. There are several issues with the windshields on the TX350 per online searches. The windshields are a laminated acrylic one that are prone to cracks requiring replacement. This is a common issue on the TX350. The windshields are on long back orders for a genuine Lexus part. We are having our second windshield replaced on XXX at the Lexus dealer. I am of the opinion that this issue should be looked into by NHTSA for a potential recall and improved manufacturing process for future original equipment and genuine Lexus replacement. Our TX350 will be available for inspection on XXX at Parker Lexus in Little Rock Arkansas. The approximate cost to replace the windshield is $1850.00. We are not claiming the windshield on our insurance for this replacement for fear of potentially being cancelled by our insurance company. Please research this issue at your earliest convenience and feel free to request more information from me if needed to help in the assessment of the problem. We look forward to hearing from you asap. We were traveling 80 miles west of Fargo ND on I94 when we heard a loud bang. It became clear immediately from the wind noise that the sunroof had broken. We did not hear or see that anything was dropped on it. Thankfully the shade was closed. We pulled off at the next possible stop to inspect and noted a large whole in the sunroof and that the glass bowed outward suggesting the explosion was an outward force rather than an item falling on the car. There were no warning lights etc. prior to this happening. This was a very frightening experience and had the shade not been closed our family including young son could have been hurt or even blinded from the falling glass. We called the dealer (we only had the car for 3 weeks) and they were able to get the car in right away to look at it but cannot guarantee the window is covered under warranty.

The electronic throttle control, vehicle speed control, and powertrain/transmission control system failed. When I press the pedal to accelerate there was no power. However I did not lose power to the vehicle as I can see the vehicle was in Drive mode but was not moving forward. I was on a decline hill and the decline helped me go downwards before a truck could hit me. This total loss of acceleration has occurred exactly 2 times within a 4-week period. The vehicle is currently located at the Lexus dealership service center and is available for immediate inspection upon request. My wife and small children were in the vehicle when the accelerator pedal went completely dead while driving. The sudden drop in speed in active traffic almost caused us to be catastrophically rear-ended by an 18-wheeler semi-truck. After one minute of zero throttle, the vehicle violently jerked, locked us into a highly dangerous 5 MPH limp mode on the road, jerked a second time, and then suddenly resumed normal speed. My family's lives were put at extreme risk. No. The vehicle is currently at the authorized Lexus dealership for inspection, but the service department states they cannot find any diagnostic trouble codes (DTCs) and have not yet reproduced the intermittent failure on a test drive. The vehicle is currently under inspection by the authorized Lexus dealership service department. It has not yet been inspected by the manufacturer's regional corporate specialists, police, or insurance. No. There were absolutely no warning lamps, check engine lights, dashboard messages, or symptoms prior to either of the two failures. The loss of acceleration happened instantly without warning. The vehicle's computer system failed to store any error codes despite the severe mechanical jerking and the sudden 5 MPH speed restriction.

- #11749022July 8, 2026STRUCTURE:BODY:DOOR:HANDLE:INTERIOR, LATCHES/LOCKS/LINKAGES:DOORS:LATCH:EMERGENCY MECHANICAL RELEASE, ELECTRICAL SYSTEM:12V/24V/48V BATTERY
On Juky 7, 2026 @ 10:30pm a catastrophic safety failure occurred involving a 2026 Lexus TX 350. The vehicle experienced a complete electrical and software failure while an occupant (my mother) was inside. All electronic door release buttons failed simultaneously, completely locking the occupant inside the vehicle. The emergency manual double-pull override mechanism on the interior door handles failed to mechanically release or open any of the doors. Concurrently, the climate control system locked on high heat, causing the cabin temperature to rise rapidly. Due to the total entrapment and extreme heat, the occupant began losing consciousness, and her blood pressure spiked to a dangerous level of 188/100. Family members outside the vehicle were unable to open the doors or bypass the electronic locks from the exterior. Emergency services were called to the scene. The local Fire Department had to perform an emergency extraction to rescue the occupant from the vehicle. The occupant required immediate medical attention for severe heat stress and a hypertensive crisis.

- #11748491July 6, 2026TIRES:TREAD/BELT
We were driving down the highway and the vehicle started shaking. I immediately slowed down as I felt we had a flat. There was notification on the screen. I moved to the right onto the shoulder. Only after we were completely stopped did I see a low tire warning come on display. I slowly got out as the traffic was heavy. I could see the outer section of the tire came apart from both sides. As we were in a dangerous location someone called the police. Two police cars pulled behind us and provided us some safety. We agreed to swap out bad tire for spare as we were in a very dangerous location and needed to get vehicle moved before someone ran into all of us. We did look at the highway to ensure no objects were in the road. I didn’t see any while driving, but we wanted to verify to protect other drivers. There also weren’t any pot-holes in highway. We inspected tire after taking it off and there were no screws, nails, objects, or damage to the center tread/sides. The tire seemed to separate fairly cleanly on both sides. Tires were Continental Cross-contact 255/45 R22 107V LX20 (Eco plus) To repeat, no Warning Lights came on vehicle before tire came apart. I have sent message to my insurance company. No one has inspected tire and it is in my possession.

- #11747786July 1, 2026FUEL/PROPULSION SYSTEM
I am reporting two separate but related fuel-system defects on my 2025 Lexus TX500h (VIN [XXX]) that raise safety concerns and appear to be widespread across Lexus TX and NX hybrid models. FUEL FILLER DOOR FAILS TO OPEN: The electronic fuel filler door intermittently fails to open when the release is activated. It sometimes requires multiple attempts, and at times will not open at all, leaving the vehicle unable to be refueled. This creates a safety risk of being stranded with no ability to add fuel, including the risk of running out of fuel on the road or being unable to refuel in an emergency. The failure appears to worsen in cold weather. This is a widely reported problem on 2022-2025 Lexus NX and TX hybrid vehicles and has been attributed to a faulty electronic fuel-door lock actuator (referenced by owners and parts suppliers as OEM part number 77030-F6020). Owners are frequently forced to use a manual emergency release located behind an access panel in the cargo area in order to refuel. FUEL TANK CANNOT BE FILLED TO CAPACITY: On a near-empty tank, the fuel pump nozzle shuts off after only approximately 8 gallons, despite the tank's rated capacity of approximately 17 gallons. Adding additional fuel requires repeated, prolonged attempts and often only a few more gallons can be added. This significantly reduces usable driving range relative to what the vehicle's fuel gauge and range estimate imply, creating a risk of unexpectedly low fuel and reduced range in situations where refueling options are limited. Other owners have reported the same behavior, and in at least one documented case a Lexus Field Technical Specialist diagnosed the cause as a deformed/defective fuel tank requiring a redesigned replacement. Both conditions are present on my vehicle. I am reporting them so that they are documented at the federal level. No crash, injury, or fire has occurred. I have also raised these issues with my Lexus dealer. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

y 2025 Lexus TX500h experienced sudden and complete AC failure at fewer than 25,000 miles, 18 months after purchase. The selling dealer diagnosed a refrigerant leak from the AC condenser and denied warranty coverage citing road debris impact damage. No photographic documentation of a discrete impact event or specific leak point was provided to support the denial. This failure is consistent with a documented, systemic design vulnerability in the Lexus TX platform. The vehicle's wide-open front grille design allows road debris direct access to the AC condenser, which sits as the frontmost component in the cooling stack with no protective barrier. Multiple NHTSA complaints document identical failures on TX platform vehicles at extremely low mileages including 580 miles and 900 miles, with dealers uniformly denying warranty coverage. Operating without AC in Texas summer heat constitutes a safety hazard, particularly for child passengers. I am requesting NHTSA investigate this as a systemic design defect pattern across the TX platform. Attached photos include: (1) photos provided by the dealer as their evidence of "impact damage" — none of which document a specific leak point or impact deformation zone; and (2) photos taken by the owner of the condenser on the Westside Lexus loaner vehicle provided during this service visit, which show comparable fin condition and debris accumulation to the denied vehicle. The loaner vehicle photos demonstrate that this condenser condition is consistent with normal operation in this environment, not discrete impact damage.

- #11745699June 22, 2026ENGINE, STRUCTURE:BODY, UNKNOWN OR OTHER
On our 2025 Lexus TX350, less than 1 year old, and under normal driving conditions, our AC stopped working suddenly. Upon inspection at the dealership, they determined a rock or road debris came through the front grill and impacted the condenser, which caused a major refrigerant leak and ac to stop working. I believe this was due to a poor design of the lower half of the front grill, it’s wide open and offers little to no protection to ac and engine components. They should have a better honeycomb or similar grill to protect components. The refrigerant leak is an environmental and road safety issue, and living in hot climates like southern Nevada, when it’s 110 degrees outside, the heat inside the vehicle with no AC is a safety issue for children, adults, etc. The dealership did not cover repairs under warranty, and quoted $3,500 to repair either with cash or submit insurance claim.

- #11744460June 16, 2026STRUCTURE:BODY:HOOD
I purchased my vehicle 2026 Lexus tx350 from Lexus of Manchester located in [XXX]. As soon as I left the lot I noticed the hood would shake violently and almost pop up which signaled the safety latch alarm. The alarm would signal multiple times throughout a highway trip. Most noticeable around 70 mph. The dealership was helpful and set up an autobody repair where they added adhesive into the hood. Unfortunately the hood flutter is still present and it’s only a matter of time before the adhesive wears down. After conducting research Lexus issued a bulletin repair acknowledging the fault in engineering regarding the aluminum light hoods. The hood is clearly to light and this is an obvious engineering defect which should signal recalls. I have read thousands of similar cases and Toyota is failing to take accountability of this problem. They need to replace the hoods instead they are refusing to acknowledge this is a recall and sneakily sending vehicles out to autobody shops. I have read the Lexus GX and TX both have the same problem. It is not safe to have a hood flutter like that it puts way to much strain on the hood safety latches and eventually they will fail. I do find it absurd and ridiculous that a brand new 70,000 SUV has such a problem. My hope is submitting this complaint it helps force Toyota do the right thing and take accountability for their engineering defect. - #11744286June 15, 2026FUEL SYSTEM, GASOLINE
The contact owns a 2026 Lexus TX 350. The contact stated that after an extended driving period, the vehicle was parked in the garage, and the contact became aware of a strong gasoline odor. The fumes caused persistent headaches that lasted two days. The contact had not sought medical attention. Upon inspection, the owner was unable to locate a visible fuel leak. The vehicle was moved to the driveway and was later towed to the dealer. After an extensive diagnostic test was performed by the dealer, the cause of the failure was not yet determined. The vehicle remained with the dealer and was not repaired. The manufacturer was notified of the failure, and the contact was provided a case number. The failure mileage was 3,563.

On two occasions while driving my hood has become open due to windy conditions coming from the Makua side of the highway as I’m driving on the Queen Kaahumanu Highway Today was the second incident except that I was heading south The first time that happened I was driving north and it occurred probably about a few months ago, but I dismissed it I know for a fact that I did not open the engine compartment I will download my front camera files and see if I can find a record

The Lexus TX 500h is equipped with a 6-speed sequential-shift automatic transmission, which is paired with the Lexus DIRECT4 all-wheel drive system. This setup utilizes a 2.4-liter turbocharged hybrid powertrain to deliver 366 horsepower and 406 lb-ft of torque. During many occasions, the transmission on the 2026 Lexus TX500h would shudder during acceleration or deacceleration. I noticed it happening around 25-40mph mostly and it has happened more than once. I have demonstrated the issue to the dealer on Wed, May 20th around 10:15am Pacific but they didn't know how to address the issue. I did a ride along with the service tech from Lexus of Pleasanton and it happened again during acceleration. The dealer noted that there were no error code when they scanned the system and no observed leaks. I have a video clip but was unable to upload it. The issue with transmission shift thump when this issue happen feels like the transmission locked up. It would shake the vehicle and lose power while accelerating before it starts to gain power back in 1-2 seconds. This causes a safety issue since that pause in acceleration thus this can be dangerous while merging onto a freeway or areas with lots of traffic. I noticed the same issue on some occasions, it would shake during deceleration as well. This issue is intermittent. Maybe Lexus/Toyota can address this with the transmission shifting update. IMO, It's a safety issue that needs to be looked and fixed by Lexus/Toyota. Dealer asked me to come back only. They reset my information screen to address a sliding driving issue where when the power is turned off on the vehicle the seats will go back and immediately go forward.

2025 Lexus TX350, approximately 27,000 miles. Vehicle exhibits rotational clicking/vibration from front drivetrain during acceleration while turning. Dealer is attributing issue to brake pad and rotor wear. Lexus TSB L-SB-0033-24 Rev1 documents a known front drive shaft assembly defect on this platform. TSB applicability check has not been performed. Requesting investigation into whether this defect extends to 2025 model year production.

To Whom It May Concern, I am writing to file a formal safety complaint regarding my 2026 Lexus TX 350 F Sport (VIN: [XXX] ), purchased from Treasure Coast Lexus in Florida in December of 2025. Shortly after purchasing the vehicle I noticed significant hood flutter at 60-70 MPH. This is visually distracting and poses a safety concern while driving. Despite three separate repair attempts at the dealership in the past two months the defect has not been resolved. Lexus corporate has since informed me that nothing further can be done to correct the issue. I am aware that this is a known issue across the Lexus TX lineup and that a related Technical Service Bulletin (L-SB-0013-25) was issued in February 2025 for the GX 550, which shares the same hood construction. I am requesting that NHTSA investigate this defect as it affects driver safety and may impact other owners of the 2025 and 2026 Lexus TX 350. Thank you for your attention to this matter.
