A small pebble or rock was thrown from another moving vehicle and it was able to get through the large holes of the grill and punctured the radiator. There isn’t adequate protection for the radiators from such common road debris.

The front grill has enlarged holes and a rock was able to penetrate the grill and hit the condenser. It broke the condenser causing a very expensive repair. I made a comprehensive claim with my insurance and they agreed that was the cause of the damage. The grill does not provide sufficient protection for the parts inside. I had no warning lamps go off, the Air conditioner simply stopped working a few days later.

The front panel of the two-panel glass moonroof spontaneously shattered, buckled and collapsed onto the panoramic roof sunshade of my 2025 Toyota Grand Highlander Hybrid without any impact or accident. I was driving at about 35-45 mph on a paved road and noticed the sound of wind coming from the car ceiling. I opened the panoramic roof sunshade slightly to see if the moonroof was open and saw that the front panel of the two-panel glass moonroof had spontaneously shattered. By the time I got home about an hour later, the moonroof glass had buckled about 1/2 inch into the air and a large chunk of the moonroof glass had collapsed onto the sunshade on the ceiling of the car. I now have a 6x10-in. hole in the moonroof glass and large, raised cracks much longer than that. The repair shop told me not to drive the car, because the broken shards of glass could damage the sunshade and get into the moonroof track. My car is available for inspection upon request. The defect in the moonroof glass clearly put my safety at risk. Had the sunshade not been closed when the glass collapsed, my car would have been full of shards of glass. I could have been hurt, and it could have caused an accident. This problem has been reported online repeatedly in Toyota Highlanders, as well as in Toyota RAV4s, resulting in a class action lawsuit with respect to the RAV4s. A service representative at the Toyota dealer saw the problem, but she could not help me because they don't handle glass. I also reported the problem to Toyota at 800-331-4331, but they have not inspected the damage. I have been informed that there are no replacement skylights for my car anywhere in the US and that it will take 1-2 months before the part will arrive. Nevertheless, Toyota has refused to pay for a rental car or cover the indoor storage I will need to obtain for my vehicle while we wait for some indeterminate time for the part to come in. There were no warnings of any kind before the skylight glass failed.

Experienced repeated axle issues, acceleration hesitation, and poor suspension performance. Dealer failed to identify or reproduce the problem despite multiple visits and diagnostic fees. Axles eventually required out-of-pocket replacement due to popping and clicking. Suspected axle defect persists, and the vehicle remains unsafe-feeling and unresolved. Requesting Toyota investigate and provide free repairs.

- #11773404October 24, 2026SERVICE BRAKES, UNKNOWN OR OTHER, VEHICLE SPEED CONTROL
Summary: While turning into my driveway, the vehicle surged forward unexpectedly as I moved my foot from the accelerator to the brake pedal. The brake pedal felt normal, and none of the vehicle’s safety systems appeared to activate. The vehicle struck my husband’s parked car and then my garage door, damaging both, and I sustained minor injuries. Details: On 10/8/26, at approximately 5:58PM, I was driving home from work and turning into my two-car driveway. I normally park to the left of my husband’s vehicle, which was parked in the driveway. As I turned in, I took my foot off the accelerator and moved it to the brake pedal, as I normally do. The brake pedal felt normal. Instead of slowing, the vehicle surged forward abruptly. No warning lights, alerts, or automatic interventions activated that I noticed. The vehicle struck my husband’s car and then continued to collide into my garage door. During the event, I was gripping the gear shifter. After the car collided into the garage, the vehicle stopped. Outcome: Both my husband’s vehicle and my garage door were damaged. I sustained back pain and a sprained right ring finger, which I believe resulted from bracing against the gear shifter during the impact. Additional information: I believe an electronic malfunction caused the vehicle to accelerate abruptly as I transitioned between pedals.

- #11773306October 23, 2026FUEL/PROPULSION SYSTEM
The vehicle’s fuel system is allowing a large amount of gas fumes to escape. The gas fumes are more noticeable on the driver side and in hotter weather. The vehicle has a significantly low MPG than advertised with some trip readings showing 11 MPG. The issue has been reported twice to the dealership and they stated that they cannot duplicate the problem but stated that they have received multiple reports from other vehicle owners of similar issues. The fuel system leak presents a risk of fire or explosion when the vehicle is stored in a confined space such as a garage. We first noticed the issue back in August or early September. I parked the vehicle in the garage and the space quickly filled with gas fumes. When I returned to the garage and the door to the dwelling was left open my home also filled with fumes. My daughter complained of the smell and stated that it was giving her a headache. I no longer park the vehicle in the garage. Since then I have noticed from time to time, the smell of gas fumes in the cabin. The vehicle is available for inspection upon request.

On XXX, at about 2:00 PM we were driving south on I95 somewhere in [XXX] when we heard a very loud noise. Thinking it was a rock hitting the windshield we looked but could see no cracks or chips so we kept driving on our trip to [XXX]. The next day it was windy and we thought it was unusually noisy inside the vehicle but we just thought it was the wind. After driving around in [XXX] we said it was unusually noisy inside so I said I would stand on the door sill to look at the roof and that’s when I saw the hole in the sunroof. That night, Sunday, XXX, I called our insurance company to report the damage. They said that an adjuster would contact us on Monday. Meanwhile I started to “Google” issues with sunroofs and there I found out about imploding glass sunroofs. On Monday, XXX, I went to the Toyota service in [XXX].and had them look at the damaged sunroof. The service advisor mentioned he had never seen this damage. He said they could replace it but would have ordered parts which could take a couple days. I said to go ahead as we were only here until Saturday. That day, Monday XXX I called the insurance company to update them. I also stopped at the Toyota dealer to talk with them. I mentioned the “Google” reports about sunroof issues. The advisor said there were no recalls or anything from Toyota on this. He did say that an insurance adjuster had contacted him and said they were very busy but would try to get there to look at the vehicle on Tuesday. We are thankful we did not have the sunroof cover open as we would have had glass all over inside the vehicle. Back pockets of the driver and passenger seats were torn at all 4 corners. Did not realize this when the car was purchased because I do not sit in the back seat and my kids have never put anything in the pockets. Obviously by just looking at the pockets you can see that it is a manufacturer defect. Toyota is blaming children or putting things to heavy in the pockets. Poor workmanship. Basically, saying that if they were to say this was poor workmanship, they would have to pay to fix. This was a purchased brand-new car. I have seen other complaints about this situation online.

- #11772339October 20, 2026FORWARD COLLISION AVOIDANCE: WARNINGS, POWER TRAIN
Incident occurred on XXX at 9:46AM PST, [XXX]. When applying brakes normally to come to a gradual stop behind another vehicle stopped at a stop sign on a slight decline, PCS collision warning lights enabled and vehicle lurched forward while my foot was on brake. This is not to be confused with automatic braking behavior as front collision warning lights in driver information center flashed red. Rather, vehicle lurched forward as if accelerating once within feet of vehicle in front. Then, vehicle rocked front to back multiple times with my foot still on the brake, as if Drive and Reverse was being quickly and intermittently engaged back and forth about 1 second apart. Had my foot not been firmly on the brake, the vehicle felt like it would travel forward and then back (colliding with vehicle in front and in back) without driver inputs. While car continued to lurch forward and then back, I shifted transmission into Park then back in Drive, and behavior stopped. There was a loud clunk as the car engaged in Drive. This has occurred 3 times over the past 2.5 years. What is even more curious, is that this occurred all 3 times at the same stop sign, which is at a slight decline. This leads me to deduce that somehow the angle of decline or other specific condition at this stop sign is somehow contributing or confusing vehicle systems, as bizarre as this sounds. AC was on during at least one preceding event but not during yesterday's event. Others have observed similar events on on-line forums, but I don't know if they have reported this given how infrequently it occurs. Vehicle has 28000+ miles, and I stop at this exact location at least 3 times per week, with behavior observed only 3 times. Statistically, it seems probable that there must be some element (angle of decline?) at this location that somehow confuses vehicle systems, triggering this dangerous unintended acceleration and reversing behavior. I am reporting a serious safety concern involving the right rear passenger door of my 2026 Toyota Grand Highlander Hybrid, which has approximately 11,000 miles. My [XXX] daughter attempted to exit the vehicle from the rear passenger seat but was unable to open the door from inside. I immediately attempted to open the door from outside to assist her, but the exterior handle also failed to release the door. After approximately seven attempts, the door finally opened. This is extremely concerning because my child was effectively trapped inside the vehicle, and I was unable to open the door to assist her. In the event of an accident, vehicle fire, or other emergency, this malfunction could prevent passengers from escaping or delay emergency responders. I have since discovered reports from other Toyota Grand Highlander owners describing similar failures, including the same right rear passenger door refusing to open from either side. My vehicle is nearly new, with only 11,000 miles, and I am concerned that this may indicate a defective door latch, lock actuator, or related mechanism. I am requesting that NHTSA investigate whether this is a broader safety defect affecting Toyota Grand Highlander vehicles. A passenger door should reliably open when properly unlocked, especially when a child is inside. This malfunction presents a serious potential risk to occupant safety. I have had my 2026 Grand Highlander Max Hybrid for only 3 months. Shortly after buying it I started noticing issues with how it shifted. It was hard for me to explain at first, but as the issue happened more it became more apparent. Periodically when driving I will be stopped at a red light or stop sign and attempt to accelerate and there is an obvious delay for a couple seconds followed by a surge of acceleration to high RPM. My foot is in a general form for acceleration at about 25 mph. This is concerning because if I pull in front of traffic timing when based on the expected acceleration, you can imagine that this is scary if I have this delay and oncoming traffic hits me. This issue is the most obvious, but there are other issues such as jerking and rough shifting at lower speeds periodically. I have taken in the car 5 times now. The first time they replicated it and said they did an update that fixed it. It did not resolve. The other times they have not been able to replicate it because it happens periodically and I can’t go without my car for several days or weeks for them to drive it to see if it does it. I have filed a issue claim and lemon law with Toyota and they just told me to go back to the dealership and denied my lemon law claim. This is a huge safety concern and Toyota isn’t doing anything about it.

- #11771331October 15, 2026FUEL/PROPULSION SYSTEM
The vehicle exhibits abnormal fuel system behavior. When the vehicle reaches a low fuel condition (approximately 40–50 miles to empty), the fuel tank only accepts about 12 gallons, even though the listed tank capacity is 17.2 gallons. After refueling, the fuel gauge displays FULL and the “Distance to Empty” reading jumps to over 360 miles, which is inconsistent with the amount of fuel added. A Toyota dealer documented this behavior on a repair order, stating that the tank accepted only 12 gallons from a low fuel condition and immediately displayed a range of 363 miles. A second repair order states that the fuel system was inspected and that the tank supposedly has a “3–5 gallon reserve,” but no technical documentation was provided to support this claim. Toyota’s customer support later provided a written response stating only that mathematically 3–5 gallons must have been in the tank, without offering any engineering explanation for why the gauge reads FULL or why the vehicle reports an inflated driving range after adding only 12 gallons. The dealer also provided documentation explaining the issue using online forums and social media posts rather than Toyota engineering documentation. Safety concern: Because the gauge displays FULL and the vehicle reports a high range when the tank has not actually been filled to capacity, the vehicle may be operating with significantly less fuel than indicated. This creates a risk of unexpected fuel starvation, hybrid system shutdown, and loss of propulsion if the vehicle runs out of fuel while the driver believes adequate range remains. Supporting documents include dealer repair orders, Toyota’s written response, a dealer explanatory response, and confirmation that the issue was escalated internally.

Dear NHTSA Vehicle Safety Team, I am writing to report a serious steering system issue to my 2024 Toyota Grand Highlander Hybrid and to request that NHTSA review this incident for potential safety concerns affecting other owners of the same vehicle model. Vehicle Information - Make and Model: 2024 Toyota Grand Highlander Hybrid - VIN: [XXX] - Mileage at the time of the issue: Approximately 66,000 miles - Component involved: Steering rack/rack-and-pinion assembly Sequence of Events 1. Steering System Failure At 65,000 miles, i started having minor issues with the steering system, but was driving fine. At 85,000 miles i had the vehicle examined at Toyota, (cause steering rack just stuck) informed me the steering rack needed to be replaced, (repair cost 3300$). 2. Vehicle Maintenance History I have maintained my vehicle in a timely manner. at always 5000 miles, during my communication with Toyota's Brand Engagement Center, they acknowledged that my maintenance history was satisfactory. 3. Previous Accident and Service Documentation I contacted Toyota's Brand Engagement Center and requested goodwill assistance with the repair because of the nature of the failure, the vehicle's mileage, and its maintenance history. Toyota ultimately declined to participate in the repair, citing the prior accident. I also requested a written explanation , but denied my request for goodwill assistance, citing the vehicle's involvement in an accident, minor rear end to my rear door and bumper, not my fault. My service documentation dated XXX, (2 months prior the accident) contains a reference to the rack-and-pinion boot and a listed amount of $54.26, this service entry and the eventual steering rack failure to be properly examined. I am not claiming that the accident or the rack-and-pinion boot necessarily caused the failure. I just found out that my model vehicle had a steering wheel recall but 2026 model, maybe has the same issues. My Toyota Grand Highlander Hybrid Limited has 16000 miles. Yesterday, 5 vehicle alerts appeared simultaneously: Check Engine, AWD system, Drive-Start control system, Intelligent clearance Sonar, and Hybrid system. The vehicle has reduced power and check engine light is illuminated. I towed the vehicle to dealership. They found damage to inverter radiator causing a leak of coolant. Dealership mentioned that this could be because of flying debris hitting the vent. I am requesting a goodwill technical review and repair of the same. Referencing to the NHTSA defect petition DP26006 where people have encountered similar issues with the grand highlanders from 2024-2026.

- #11771001October 13, 2026STEERING, WHEELS, LANE DEPARTURE: ASSIST
I experienced a serious safety incident involving my 2025 Toyota Grand Highlander with approximately 14,000 miles. While driving on a straight interstate highway at approximately 70 mph, surrounded by other vehicles, the steering wheel suddenly made two to three sharp, forceful jerking movements to the left. Afterward, the steering wheel remained approximately 20 degrees off-center to the left. All lane departure assistance sensors were active during the incident. This unexpected steering behavior created a significant safety hazard. At highway speed, I could have lost control of the vehicle, departed my lane, or collided with surrounding traffic. I was able to reach my destination, but the steering wheel remained crooked. Immediately afterward, I took the vehicle to an authorized Toyota dealership for inspection and repair. The technician reported that the left rear wheel was significantly out of alignment and that the left rear alignment eccentric lock nut was loose. According to the technician, this caused excessive wear on the inside tread of the left rear tire. The dealership also performed a vehicle health check to identify any system faults. My vehicle has always been serviced by authorized Toyota dealerships. At the last service appointment, at approximately 10,000 miles, no related problems were identified. The technician confirmed that no alignment-related work had been performed during that visit. The vehicle had no previous similar issues. I am concerned that the loose alignment eccentric lock nut and resulting severe misalignment may indicate a mechanical defect that could compromise vehicle stability and steering safety. The sudden steering movements at highway speed presented a serious risk to me, my passengers, and other motorists. I respectfully request that NHTSA investigate this incident and determine whether a safety defect exists in the 2025 Toyota Grand Highlander involving the rear suspension alignment component, eccentric locking, sensors.

Vehicle is experiencing hesitation, stuttering, and almost stalling at low speeds when the engine is running (it is a hybrid). Sometimes when pressing the accelerator when the vehicle experiences this hesitation, the vehicle does not move forward and bogs down, almost stalling out. This has happened twice when trying to quickly pull into traffic, and has almost resulted in an accident, since the vehicle failed to move forward when commanded. The dealership was able to reproduce the issue but said there is no fix available. There are no warning lamps or messages associated with this condition.

On 9/26 at about 10:15am, my husband and I were fully stopped at a red light with his foot on the brake. Without him touching the gas pedal, the car suddenly surged forward as if it had been accelerated. It stopped for a moment, then surged forward again. Because he was holding the brake, the car didn’t move much, but each surge caused a hard jolt that felt like being rear-ended. No other vehicle hit us.
